中药穴位贴敷联合心理干预治疗脾肾两虚型早期先兆流产的临床效果观察

摘　　要：目的:探讨采用中药穴位贴敷结合心理干预治疗早期先兆流产(脾肾两虚型)的临床效果,并分析对Th1/Th2细胞因子平衡的影响。方法:选取2021年1月—2023年1月瑞昌市中医医院收治的早期先兆流产患者80例,属脾肾两虚型作为研究对象,按照随机数字表法分为对照组40例(西药常规治疗)、观察组40例(在对照组的基础上予以中药穴位贴敷联合心理干预)。观察两组患者临床疗效、激素水平、焦虑、抑郁、心理症状、Th1/Th2细胞因子水平。结果:治疗后,观察组患者总有效率、β-人绒毛膜促性腺激素(β-hCG)、孕酮(P)、雌二醇(E_(2))、白细胞介素-4(IL-4)水平均高于对照组,白细胞介素-2(IL-2)、干扰素-γ(IFN-γ)、焦虑自评量表(SAS)、抑郁自评量表(SDS)、症状自评量表(SCL-90)均较对照组低,差异均有统计学意义(P<0.05)。结论:脾肾两虚型早期先兆流产患者应用中药穴位贴敷联合心理干预可有效提高治疗效果,改善激素水平,调节Th1/Th2细胞因子水平,且改善心理状态。Objective:To explore the clinical effect of traditional Chinese medicine acupoint application combined with psychological intervention in the treatment of early threatened miscarriage of spleen and kidney deficiency type,and to analyze its impact on Th1/Th2 cytokines.Method:Eighty patients with early threatened miscarriage admitted to Ruichang Traditional Chinese Medicine Hospital from January 2021 to January 2023,belonging to the spleen kidney deficiency type,were selected as the research subjects.According to the random number table method,there were 40 patients in the control group(treated with conventional western medicine)and 40 patients in the treatment group(treated with traditional Chinese medicine acupoint application combined with psychological intervention on the basis of the control group).The clinical effect,hormone levels,anxiety,depression,psychological symptoms,and Th1/Th2 cytokine levels of the two groups of patients were observed.Result:After treatment,the total effect rate,the levels ofβ-human chorionic gonadotropin(β-hCG),progesterone(P),estradiol(E_(2)),interleukin-4(IL-4)in the observation group were higher than those in the control group,the levels of interleukin-2(IL-2),interferon-γ(IFN-γ),self-rating anxiety scale(SAS),self-rating depression scale(SDS),and symptom checklist-90(SCL-90)in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).Conclusion:The application of traditional Chinese medicine acupoint application combined with psychological intervention in patients with early threatened miscarriage of spleen kidney deficiency type can effectively improve their treatment effect,improve their hormone levels,regulate Th1/Th2 cytokine levels,and improve their psychological state.
