微型钢板内固定和空心螺钉内固定治疗后踝关节骨折的效果比较

摘　　要：目的:观察在后踝关节骨折的临床治疗中,微型钢板内固定和空心螺钉内固定的应用效果。方法:回顾性选取2022年10月—2024年5月江苏大学附属武进医院收治的后踝关节骨折患者62例,按照不同治疗术式分为两组,对照组(31例)采用空心螺钉内固定,研究组(31例)采用微型钢板内固定,对比两组临床疗效。结果:术后3个月,研究组治疗优良率和美国足踝外科协会(AOFAS)踝与后足功能评分均高于对照组,且并发症发生率、术后7 d视觉模拟评分法(VAS)评分,以及术后3 d的丙二醛(MDA)、醛固酮(ALD)、皮质醇(Cor)水平均低于对照组,差异均有统计学意义(P<0.05),两组术中出血量、骨折愈合时间和住院时间比较,差异均无统计学意义(P>0.05)。结论:在后踝关节骨折的临床治疗中,相较于空心螺钉内固定,微型钢板内固定能够促进患者术后足踝功能恢复,降低并发症发生风险,缓解创伤应激刺激,安全性更高。Objective:To observe the application effects of mini steel plate internal fixation and hollow screw internal fixation in the clinical treatment of posterior ankle joint fractures.Method:A retrospective study was conducted on 62 patients with posterior ankle joint fractures admitted to Wujin Hospital Affiliated to Jiangsu University from October 2022 to May 2024.They were divided into two groups according to different treatment procedures.The control group(31 cases)received hollow screw internal fixation,while the study group(31 cases)received mini steel plate internal fixation.The clinical efficacy of the two groups was compared.Result:At 3 months after operation,the excellent and good rate of treatment,American orthopaedic foot and ankle society(AOFAS)ankle and hindfoot function score of the study group were higher than those of the control group;the incidence of complications,visual analogue scale(VAS)score at 7 days after operation,and malondialdehyde(MDA),aldosterone(ALD)and cortisol(Cor)levels at 3 days after operation were lower than those in the control group,the differences were statistically significant(P<0.05).The intraoperative blood loss,fracture healing time and hospitalization time of the two groups were compared,the differences were not statistically significant(P>0.05).Conclusion:In the clinical treatment of posterior ankle fractures,compared with hollow screw internal fixation,mini steel plate internal fixation can promote postoperative ankle function recovery,reduce the risk of complications,alleviate traumatic stress stimulation,and have higher safety.
