摘　　要：目的:探讨三维能量多普勒(three-dimensional power Doppler,3D-PD)联合断层超声显像(tomographic ultrasound imaging,TUI)对子宫腔粘连的诊断价值。方法:选择2022年5月—2024年4月于赣州市妇幼保健院就诊的疑诊子宫腔粘连患者103例。所有患者均行宫腔镜检查及经阴道三维超声检查。根据患者是否有子宫腔粘连分为试验组(n=53,确诊为子宫腔粘连)与对照组(n=50,无子宫腔粘连)。记录患者子宫内膜下血流参数数据,观察3D-PD联合TUI诊断子宫腔粘连及其分度的诊断价值。结果:试验组子宫内膜容积(endometrial volume,EV)、血管化血流指数(vascularization-flow index,VFI)、血流指数(flow index,FI)、血管化指数(vascularization index,VI)、子宫内膜厚度(endometrial thickness,ET)均小于对照组(P<0.05)。轻度组VI、FI、VFI、ET均大于中度组,中度组均大于重度组(P<0.05);轻度组EV大于重度组(P<0.05)。以宫腔镜检查结果为金标准,TUI诊断子宫腔粘连程度的准确度为84.91%(45/53),Kappa值为0.762(P<0.001);TUI联合3D-PD诊断子宫腔粘连程度的准确度为92.45%(49/53),Kappa值为0.883(P<0.001)。结论:TUI联合3D-PD对子宫腔粘连程度的诊断价值高,与宫腔镜检查有较高的一致性。Objective:To investigate the diagnostic value of three-dimensional power Doppler(3D-PD)combined with tomographic ultrasound imaging(TUI)in intrauterine adhesion.Method:A total of 103 patients with suspected intrauterine adhesion treated in Ganzhou Women and Children's Health Care Hospital from May 2022 to April 2024 were selected were selected.All patients underwent hysteroscopy and transvaginal three-dimensional ultrasonography.Patients were divided into experimental group(n=53,confirmed intrauterine adhesion)and control group(n=50,no intrauterine adhesion)according to whether they had intrauterine adhesion.The data of subendometrial blood flow parameters were recorded,and the diagnostic value of 3D-PD combined with TUI in intrauterine adhesion and its grading were observed.Result:Endometrial volume(EV),vascularization-flow index(VFI),flow index(FI),vascularization index(VI)and endometrial thickness(ET)in the experimental group were smaller than those in the control group(P<0.05).VI,FI,VFI and ET in mild group were higher than those in moderate group,and those in moderate group were higher than those in severe group(P<0.05).EV in mild group was higher than that in severe group(P<0.05).Using hysteroscopy results as the gold standard,the accuracy of TUI in diagnosing the degree of uterine adhesion was 84.91%(45/53),and the Kappa value was 0.762(P<0.001).The accuracy of TUI combined with 3D-PD in diagnosing the degree of uterine adhesion was 92.45%(49/53),and the Kappa value was 0.883(P<0.001).Conclusion:TUI combined with 3D-PD has high diagnostic value for intrauterine adhesion,and has high consistency with hysteroscopy.
