摘  要:目的:探究步态分析早期识别膝反张异常活动度对膝关节DR影像学及生物力学的影响。方法:选择2023年3月—2024年9月河北医科大学第一医院的50例受试者进行观察,通过步态分析测定膝关节屈伸角度,根据膝关节是否反张,分为反张观察组和无反张对照组,各25例。观察比较两组股胫角、胫骨角、股骨角、间隙夹角及膝反张角度(无反张对照组为支撑相膝关节伸展位的角度)参数,并进行相关性分析。结果:反张观察组膝反张角度、股胫角、股骨角、间隙夹角均高于无反张对照组,差异均有统计学意义(P<0.05);两组胫骨角比较,差异无统计学意义(P>0.05)。相关性分析显示,膝反张角度与股骨角、股胫角存在显著负相关(r=-0.428、-0.419,P=0.002、0.002);间隙夹角与股胫角、股骨角存在正相关(r_(s)=0.296、0.352,P=0.037、0.012),与胫骨角存在负相关(r_(s)=-0.343,P=0.015)。结论:膝关节反张程度越大,股胫角越大,膝关节间隙夹角越大;膝反张患者股骨角、股胫角、间隙夹角大于正常人群;间隙夹角与股胫角、股骨角存在相关性。Objective:To explore the influence of early recognition of abnormal knee motion in gait analysis on DR imaging and biomechanics of knee joint.Method:A total of 50 subjects in the First Hospital of Hebei Medical University from March 2023 to September 2024 were selected for observation.The knee flexion and extension angle were measured by gait analysis.According to whether the knee joint was inverted,the subjects were divided into the observation group with recurvatum and the control group without recurvatum,with 25 cases in each group.The parameters of femoro-tibial angle,tibial angle,femur angle,interstitial angle and knee extension angle(the angle of knee extension in support phase was observed in control group without recurvatum)were observed and compared between the two groups.The correlation analysis was conducted.Result:The angle of knee inversion,femoro-tibial angle,femur angle and interstitial clamp in the observation group with recurvatum were higher than those in the control group without recurvatum,the differences were statistically significant(P<0.05).There was no significant difference in tibial angle between the two groups(P>0.05).Correlation analysis showed that there was a significant negative correlation between knee inversion angle and femoral angle and femoral tibial angle(r=-0.428,-0.419,P=0.002,0.002).The interstitial angle was positively correlated with femoral and tibial angles(r_(s)=0.296,0.352,P=0.037,0.012),and negatively correlated with tibial angles(r_(s)=-0.343,P=0.015).Conclusion:The greater the degree of knee recurvatum,the greater the femoral tibial angle and the greater the angle of knee joint space.Femoral angle,femoral tibial angle and interstitial angle in patients with knee recurvatum are larger than those in normal population.The interstitial angle was correlated with femoro-tibial angle and femur angle.
