B超引导下经会阴前列腺射频消融术联合中药通精煎治疗特殊前列腺增生患者的临床疗效研究

摘　　要：目的探讨特殊前列腺增生患者采用B超引导下经会阴前列腺射频消融术联合中药通精煎治疗的疗效。方法选取2023年6月至2024年10月昆山市中西医结合医院收治的特殊前列腺增生患者50例,按照双盲原则分为对照组(25例)和观察组(25例)。对照组予以B超引导下经会阴前列腺射频消融术治疗,观察组在对照组的基础上加用中药通精煎治疗。比较两组治疗有效率,治疗前后最大尿流率(Qmax)、剩余尿量(PRV)、前列腺体积(PV)、国际前列腺症状评分(IPSS)、前列腺生活质量评分(QOL)变化。结果观察组治疗有效率高于对照组(P<0.05)。治疗后两组Qmax比治疗前上升,PRV、PV比治疗前下降,且观察组变化幅度比对照组大(P<0.05)。治疗后两组IPSS、QOL评分比治疗前下降,且观察组变化幅度比对照组大(P<0.05)。结论B超引导下经会阴前列腺射频消融术联合中药通精煎治疗特殊前列腺增生的疗效更突出,可以更好改善患者临床症状和生活质量。Objective To investigate the curative effect of B-ultrasound-guided perineal prostate radiofrequency ablation combined with Tongjing Decoction for patients with special prostatic hyperplasia.Methods Fifty patients with special prostatic hyperplasia treated in Kunshan Hospital of Integrated Traditional Chinese and Western Medicine from June 2023 to October 2024 were selected and divided into control group(25 cases)and observation group(25 cases)according to the principle of double blindness.The control group was treated with radiofrequency ablation of perineal prostate guided by B-ultrasound,and the observation group was treated with Tongjing decoction on the basis of the control group.The effective rate,maximum urine flow rate(Qmax),residual urine volume(PRV),prostate volume(PV),International Prostate Symptom score(IPSS)and prostate quality of life score(QOL)before and after treatment were compared between the two groups.Results The effective rate of the observation group which was higher than that of the control group(P<0.05).After treatment,Qmax increased,PRV and PV decreased,and the change range of observation group was larger than that of control group(P<0.05).After treatment,IPSS and QOL scores of the two groups were lower than those before treatment,and the change range of the observation group was greater than that of the control group(P<0.05).Conclusions Radiofrequency ablation of perineal prostate guided by B-ultrasound combined with Tongjing Decoction is more effective in the treatment of special prostatic hyperplasia,which can better improve the clinical symptoms and quality of life of patients.
