摘 要:目的探讨血清C肽联合糖化血红蛋白(HbA1c)诊断糖尿病的临床价值。方法选取2022年7月至2024年6月福建省闽侯上街王国飞内科诊所就诊的90例糖尿病患者为对象(糖尿病组),以同期90名健康者为对照组。入组受试者均接受血清C肽、HbA1c检测,比较两组血清C肽与HbA1c等临床指标。利用受试者工作特征(ROC)曲线分析C肽、HbA1c联合检测诊断糖尿病的价值。结果糖尿病组血清C肽低于对照组,HbA1c、FPG、2 hPG高于对照组(P<0.05)。Logistic回归分析结果显示血清C肽(OR 5.249)、HbA1c(OR 4.660)为影响糖尿病发生的独立危险因素(P<0.05)。血清C肽为1.32μg/L时,其诊断糖尿病的AUC、灵敏度、特异度分别为0.791(95%CI 0.724~0.848)、86.67%、72.22%;HbA1c为7.20%时,其诊断糖尿病的AUC、灵敏度、特异度分别为0.827(95%CI 0.764~0.880)、77.78%、83.33%;联合检测诊断糖尿病的AUC、灵敏度、特异度分别为0.890(95%CI 0.835~0.932)、94.44%、67.78%。联合检测诊断糖尿病的AUC高于血清C肽、HbA1c(P<0.05)。结论血清C肽、HbA1c诊断糖尿病的价值均较高,二者联合应用可提高对糖尿病的诊断效能。Objective To investigate the clinical value of serum C-peptide combined with glycosylated hemoglobin(HbA1c)in the diagnosis of diabetes mellitus.Methods A total of 90 patients with diabetes treated in Guofei Internal Medicine Clinic,Minhou City,Fujian Province from July 2022 to June 2024 were selected as subjects(diabetes group),and 90 healthy subjects were selected as control group during the same period.All enrolled subjects received serum C-peptide,HbA1c and test,and compared the clinical indexes of serum C-peptide and HbA1c between the two groups.Receiver operating characteristic(ROC)curve was used to analyze the value of C-peptide and HbA1c combined detection in the diagnosis of diabetes mellitus.Results Serum C-peptide in diabetes mellitus group was lower than that in control group,HbA1c,FPG and 2 hPG were higher than that in control group(P<0.05).Logistic regression analysis showed that serum C-peptide(OR 5.249)and HbA1c(OR 4.660)were independent risk factors for diabetes mellitus(P<0.05).When serum C-peptide was 1.32μg/L,the AUC,sensitivity and specificity for the diagnosis of diabetes mellitus were 0.791(95%CI 0.724-0.848),86.67%and 72.22%,respectively.When HbA1c was 7.20%,the AUC,sensitivity and specificity were 0.827(95%CI 0.764-0.880),77.78%and 83.33%,respectively.The AUC,sensitivity and specificity of combined detection were 0.890(95%CI 0.835-0.932),94.44%and 67.78%,respectively.The AUC of diabetes mellitus was higher than that of serum C-peptide and HbA1c(P<0.05).Conclusions The value of serum C-peptide and HbA1c in the diagnosis of diabetes mellitus is high,and the combined application of the two can improve the diagnostic efficiency of diabetes mellitus.
