舒芬太尼静脉复合麻醉比较罗哌卡因硬膜外麻醉对乳腺手术麻醉效果的差异性分析

摘　　要：目的对比舒芬太尼复合静脉麻醉和罗哌卡因硬膜外麻醉应用在乳腺手术当中所发挥的麻醉效果差异。方法将2024年1月至2024年9月来朝阳市第二医院麻醉科接受治疗的89例乳腺手术麻醉患者经过随机分组方法分为观察组(45例)和对照组(44例)。对照组采用罗哌卡因硬膜外麻醉,观察组通过舒芬太尼静脉复合麻醉,比较两组的麻醉优良率、血气指标、麻醉苏醒指标和不良反应。结果①观察组的麻醉优良率高于对照组(P<0.05);②T0时,两组的心率和平均动脉压差异无统计学意义(P>0.05),T1、T2时,观察组的心率和平均动脉压比对照组更稳定(P<0.05);③观察组的导管拔除时间、苏醒时间、定向力恢复时间均比对照组短(P<0.05);④观察组的麻醉药物不良反应发生率低于对照组(P<0.05)。结论相比罗哌卡因硬膜外麻醉而言,将舒芬太尼静脉复合麻醉应用于乳腺手术麻醉当中可有效提高麻醉优良率,稳定患者的心率和平均动脉压,促进麻醉的恢复,并且提高麻醉的安全性,有助于患者的手术开展和术后的恢复。Objective To compare the differences in the anesthesia effects of sufentanyl compound intravenous anesthesia and ropicaine epidural anesthesia in breast surgery.Methods A total of 89 breast surgery anesthesia patients who came to the anesthesiology department of the Second Hospital of Chaoyang City from January 2024 to September 2024 were randomly grouped and divided into observation groups(45 cases)and control groups(44 cases).The control group used ropicaine epidural anesthesia,and the observation group through intravenous compound anesthesia of sufentanyl.Compared the excellent rate of anesthesia,blood gas index,anesthesia awakening index and adverse reactions of the two groups.Results(1)Compared to the control group,the observation group exhibited a higher anesthesia success rate(P<0.05).(2)At the baseline measurement(T0),no statistically significant difference was observed in heart rate and mean arterial pressure between the two groups(P>0.05).Nevertheless,at T1 and T2 time points,the observation group demonstrated more stable heart rate and mean arterial pressure compared to the control group(P<0.05).(3)The duration for catheter removal,awakening,and recovery of orientation in the observation group was shorter than that of the control group(P<0.05).(4)The observation group had a lower incidence of adverse anesthetic reactions compared to the control group(P<0.05).Conclusions Compared with ropicaine epidural anesthesia,the application of sufentanil intravenous composite anesthesia in breast surgery can effectively improve the excellent rate of anesthesia,stabilize the patient’s heart rate and average arterial pressure,promote the recovery of anesthesia,and improve the safety of anesthesia,which is conducive to the patient’s surgical and postoperative recovery.
