摘　　要：目的:对淋巴细胞亚群和细胞因子检测在脓毒症并发多器官功能障碍综合征(MODS)中的诊断价值进行分析。方法:选择2020年1月至2023年1月在本院确诊的105例脓毒症的患者参与研究,根据是否发生MODS分为发生组与未发生组,检测患者外周血淋巴细胞亚群和细胞因子的含量,分析淋巴细胞亚群及细胞因子检测在脓毒症并发MODS中的诊断效能。结果:CD3^(+)CD19^(-)T淋巴细胞诊断脓毒症并发MODS的AUC值为0.887,CD3^(+)CD4^(+)T淋巴细胞诊断脓毒症并发MODS的AUC值为0.700,CD3^(+)CD8^(+)T淋巴细胞诊断脓毒症并发MODS的AUC值为0.565;白介素-2(IL-2)、IL-6、IL-10、肿瘤坏死因子-α(TNF-α)诊断脓毒症并发MODS的AUC值分别为0.626、0.786、0.750、0.540。结论:淋巴细胞亚群和细胞因子检测在脓毒症并发MODS中具有一定的诊断价值,其中淋巴细胞亚群中CD3^(+)CD19^(-)T淋巴细胞诊断灵敏度较高,细胞因子IL-6诊断特异度较高。Objective:To analyze the diagnostic value of lymphocyte subsets and cytokine detection in sepsis complicated with multiple organ dysfunction syndrome(MODS).Methods:105 patients with sepsis diagnosed in our hospital from January 2020 to January 2023 are selected as the study objects,and are divided into the developing group and the non-developing group according to whether they have MODS.The diagnostic value of the lymphocyte subsets and cytokines in sepsis complicated with MODS is explored through the determination of lymphocyte subsets and cytokines in peripheral blood.Results:The AUC values of CD3^(+)CD19^(-)T lymphocytes for diagnosis of sepsis complicating MODS are 0.887,the AUC values of CD3^(+)CD4^(+)T lymphocytes for diagnosis of sepsis complicating MODS are 0.700,and the AUC values of CD3^(+)CD8^(+)T lymphocytes for diagnosis of sepsis complicating MODS are 0.565,respectively;the AUC values of Interleukin-2(IL-2)、IL-6、IL-10、tumor necrosis factor(TNF-α)AUC values for the diagnosis of sepsis complicating MODS are 0.626,0.786,0.750,and 0.540,respectively.Conclusion:The detection of lymphocyte subsets and cytokines has certain clinical value in sepsis complicated with MODS,among which CD3^(+)CD19^(-)T lymphocytes in lymphocyte subsets have higher diagnostic sensitivity and IL-6 has higher diagnostic specificity.
