摘 要:目的:评估联合使用腹部线圈和脊柱线圈对腰椎磁共振成像(MRI)图像质量及扫描效率的影响。方法:本研究纳入34例在我院接受腰椎MRI检查的患者,随机分成两组:A组使用单一脊柱线圈,B组联合使用腹部线圈和脊柱线圈。记录每个序列射频能量的比吸收率(SAR),采用配对t检验比较信噪比(SNR),Mann⁃Whitney U检验比较图像的对比噪声比(CNR),使用秩和检验评估图像主观评分差异,并分析腰椎椎体脂肪分数、腰部的体厚与图像SNR之间的相关性。结果:与单独使用脊柱线圈相比,联合使用腹部线圈和脊柱线圈在T2_tse_dixon_sag序列上使腰椎椎间盘SNR提高了85.3%,SAR降低8%;在T2_tse_tra序列上,SNR提高了80.2%,SAR保持不变。整体SNR平均提高了73%,且所有评估结构中差异具有统计学意义(P<0.001)。主观评分差异同样显著(P=0.002)。腰椎椎体脂肪分数与图像SNR呈正相关,而患者体厚与图像SNR呈负相关。在相同SNR的条件下,B组矢状位和横断位扫描时间分别缩短了56 s和41 s,总扫描时间减少了97 s。结论:尽管两种线圈使用方法均可获得可用于诊断的图像,但联合使用腹部线圈和脊柱线圈不仅能提高图像的SNR,还能在相同SNR条件下缩短扫描时间,并降低SAR值,使扫描更安全。这一发现为临床腰椎MRI检查提供了一种改进的扫描方案。Objective:To evaluate the effect of combined abdominal coil and spinal coil on image quality and scanning efficiency of lumbar magnetic resonance imaging(MRI).Methods:This study included 34 patients undergoing lumbar MRI in our hospital and were randomly assigned to two groups:Group A with a single spinal coil and Group B with a combination of abdominal coil and spinal coil.The specific absorption rate(SAR)of radiofrequency energy for each sequence was recorded,comparing the signal⁃to⁃noise ratio(SNR)by paired t⁃test,contrast⁃to⁃noise ratio(CNR)of images compared by Mann⁃Whitney U test,evaluated the image subjective score differences using the rank sum test,and analyzed the correlation between the lumbar vertebral body fat fraction,body thickness of the waist,and the image SNR.Results:Compared with spinal coil alone,the combination of abdominal coil and spinal coil on T2_tse_dixon_sag sequence increased the SNR of lumbar intervertebral disc by 85.3%and decreased the SAR by 8%.On the T2_tse_tra sequence,SNR increased by 80.2%and SAR remained unchanged.The overall SNR improvement was averaged by 73%,and the difference was statistically significant across all of the evaluated structures(P<0.001).The difference in subjective scores was also significant(P=0.002).Lumbar vertebral fat fraction was positively correlated with image SNR,while patient body thickness was negatively correlated with image SNR.With the same SNR,the scanning time of sagittal position and transverse position in group B is shortened by 56 s and 41 s respectively,and the total scan time decreased by 97 s.Conclusion:Although both methods of coil use can obtain diagnostic images,the combination of abdominal coil and spine coil can not only improve the SNR of the image,but also shorten the scan time and reduce the SAR value under the same SNR conditions,making the scan safer.This finding provides an improved scanning protocol for clinical lumbar spine MRI examinations.
