摘 要:目的:探讨尿酸/肌酐比值(uric acid/creatinine,UA/Cr)水平与高血压患病率的相关性。方法:本研究为大型横断面研究,纳入2019年6月至2024年6月就诊于哈尔滨二四二医院的正常体质量(BMI:18.5~24.9kg/m2)人群2863名,平均年龄为(62.7±9.9)岁。根据UA/Cr四分位进行分组(组1:≤3.4,组2:3.4~4.3,组3:4.3~5.2;组4:>5.2),采用Logistic回归分析探讨UA/Cr水平高血压患病率的相关性,并按年龄、性别、糖尿病及估算eGFR进行亚组分析。使用限制性立方样条(restricted cubic spline,RCS)函数模型评估UA/Cr水平与高血压患病率的非线性关系。结果:本研究中高血压组人群共373例,非高血压组人群共2490例。多因素Logistic回归分析表明,随着UA/Cr增加,高血压的患病率显著增加(组4 vs.组1:OR=2.08,95%CI:1.32~3.29,P=0.002)。亚组分析显示,在eGFR≥60 mL·min·^(-1)1.73m^(-2)亚组中,随UA/Cr比值增加,高血压风险显著增加,而在eGFR<60 mL·min·^(-1)1.73m^(-2)亚组中,各组间无显著差异(交互作用P=0.002)。RCS曲线表明UA/Cr水平与高血压患病率呈单调递增关系(非线性P=0.951)。结论:在正常体质量人群中,UA/Cr比值升高与高血压患病率增加显著相关,且在高eGFR水平的人群中相关性更强。Objective:To investigate the association between uric acid/creatinine ratio(UA/Cr)and the prevalence of hypertension.Methods:This large-scale cross-sectional study enrolled 2863 normal weight(BMI:18.5~24.9kg/m2)individuals who attended Harbin 242 Hospital from June 2019 to June 2024.The mean age was(62.7±9.9)years.Individuals were divided into four groups based on the UA/Cr quartiles(Group 1:≤3.36,Group 2:3.36-4.31,Group 3:4.31-5.21;Group 4:>5.21).Logistic regression analysis was used to explore the relationship between UA/Cr levels and hypertension prevalence.Subgroup analyses were performed based on age,sex,diabetes status,and estimated glomerular filtration rate(eGFR).A restricted cubic spline(RCS)function model was employed to assess the non-linear relationship between UA/Cr levels and hypertension prevalence.Results:The study population comprised 373 hypertensive and 2,490 nonhypertensive individuals.Multivariate logistic regression analysis revealed that increasing UA/Cr levels were significantly associated with higher hypertension prevalence(4th quartile vs.1st quartile:OR=2.08,95%CI:1.32-3.29,P=0.002).Subgroup analysis showed a significant increase in hypertension risk with rising UA/Cr ratios in the eGFR≥60 mL·min·^(-1)1.73m^(-2))subgroup,while no significant differences were observed in the eGFR<60 mL·min·^(-1)1.73m^(-2))subgroup(interaction P=0.002).The RCS curve demonstrated a monotonically increasing relationship between UA/Cr levels and hypertension prevalence(non-linearity P=0.951).Conclusions:In individuals with normal body weight,elevated UA/Cr ratios are significantly associated with increased hypertension prevalence,with a stronger correlation observed in populations with higher eGFR levels.
