摘 要:目的探讨基于运动医学对量化训练联合关节镜治疗膝关节半月板损伤的疗效和对生活质量的影响。方法前瞻性研究2021年1月—2023年12月联勤保障部队第九二二医院骨科收治的96例膝关节半月板损伤患者。男性56例,女性40例;年龄27~68岁,平均47.7岁;右膝72例,左膝24例;外侧半月板损伤60例,内侧半月板损伤36例。按照随机数字表法分为量化训练组和对照组,各48例。两组均采用关节镜治疗,量化训练组采取量化训练,对照组采取常规训练。比较两组Lysholm膝关节功能评分、健康调查简表(SF-36)、临床疗效。结果量化训练组训练12周Lysholm膝关节功能总分显著高于对照组[(80.02±8.94)分vs.(62.50±7.12)分,P<0.05]。量化训练组训练12周的生理职能、生理功能、情感职能、日常活动、精神健康、总体健康、社会功能、躯体疼痛均显著高于对照组[(78.56±7.13)分vs.(74.24±6.21)分、(80.13±6.37)分vs.(73.46±6.22)分、(79.89±8.26)分vs.(75.83±7.35)分、(82.46±7.35)分vs.(70.12±6.36)分、(76.29±7.25)分vs.(68.45±6.29)分、(79.45±7.68)分vs.(72.15±6.72)分、(79.21±7.75)分vs.(69.05±6.46)分、(78.32±7.31)分vs.(64.70±6.24)分,P均<0.05]。训练12周,量化训练组的总有效率显著高于对照组[91.7%(44/48)vs.75.0%(36/48),P<0.05]。结论基于“运动医学”对关节镜治疗膝关节半月板损伤患者采取量化训练,可显著提高患者膝关节功能、生活质量和临床疗效。Objective To assess the impact of sports medicine-based quantitative exercises for patients with arthroscopic surgery for meniscus injuries.Methods Ninety-six patients with meniscus injuries admitted to our department from Jan.2021 to Dec.2023 were prospectively selected for this study.Among them,there were 56 males and 40 females aged 27-68(mean 47.7)years.Seventy-two cases involved the right knee,and 24 the left knee,with 60 lateral and 36 medial meniscus injuries.All patients received arthroscopic treatment.Using a random number table,patients were randomly divided into quantitative exercises group and control group(routine rehabilitation exercises)with 48 cases in each group.Patients in the quantitative exercises group were initially graded as excellent,good,fair and poor based on the Lysholm knee score and individualized strength of exercise plans were arranged for each subgroup.The Lysholm knee score,MOS 36-item short form health survey(SF-36),and clinical outcomes were compared between the two groups.Results After 12 weeks of exercises,the Lysholm knee score was significantly higher in the quantitative exercises group than in the control group(80.02±8.94 vs.62.50±7.12,P<0.05).Additionally,the SF-36 scores were also much better in the quantitative exercises group,for all subitems of physical role(78.56±7.13 vs.74.24±6.21),physical function(80.13±6.37 vs.73.46±6.22),emotional role(79.89±8.26 vs.75.83±7.35),daily activities(82.46±7.35 vs.70.12±6.36),mental health(76.29±7.25 vs.68.45±6.29),general health(79.45±7.68 vs.72.15±6.72),social function(79.21±7.75 vs.69.05±6.46)and bodily pain(78.32±7.31 vs.64.70±6.24),all P<0.05.Moreover,the total effective rate was significantly higher in the quantitative exercises group(91.7%vs.75.0%,P<0.05).Conclusion Sports medicine-based quantitative exercises can greatly improve the knee function,quality of life,and clinical efficacy of meniscus injury patients after arthroscopic surgery.
