Lp-PLA2、Hcy联合CRP在乙肝妊娠患者中诊断价值的临床研究

摘　　要：目的:研究脂蛋白相关磷脂酶A2(Lp-PLA2)、同型半胱氨酸(Hcy)联合C反应蛋白(CRP)在乙肝妊娠患者中的诊断价值。方法:选取2022年5月-2023年12月我院妇产科收治的乙肝表面抗原(HBsAg)为阳性的妊娠患者103例;根据肝功能是否存在异常将患者分成乙肝组(丙氨酸氨基转移酶(ALT)>40 U/L,n=45)和携带组(ALT≤40U/L,n=58),另选同期体检健康的孕妇100例作为健康组;比较三组Lp-PLA2、Hcy、CRP水平差异;应用Logistic回归分析Lp-PLA2、Hcy、CRP与乙肝妊娠相关性;受试者工作特征曲线,(ROC),分析Lp-PLA2、Hcy、CRP诊断乙肝妊娠的价值。结果:Lp-PLA2、Hcy、CRP水平在乙肝组、携带组和健康组之间存在显著差异(P<0.05),其中乙肝组携带组健康组(P均<0.05);Logistic回归显示,Lp-PLA2、Hcy、CRP与乙肝妊娠均显著相关,是乙肝妊娠的预测因素(P均<0.05);ROC分析显示,Lp-PLA2、Hcy、CRP单独诊断乙肝妊娠的AUC分别为0.923、0.928、0.971,联合诊断乙肝妊娠的AUC为0.972,高于单独指标。结论:Lp-PLA2、Hcy、CRP与乙肝妊娠相关密切,在乙肝妊娠患者中明显升高,可作为诊断该疾病的辅助指标。Objective:The diagnostic value of lipoprotein-associated phospholipase A2(Lp-PLA2)and homocysteine(Hcy)combined with C-reactive protein(CRP)in hepatitis B pregnant patients was investigated.Methods:Selected 103 pregnant patients with positive hepatitis B surface antigen(HBsAg)admitted to the obstetrics and gynecology department of our hospital from May 2022 to December 2023;Patients were divided into hepatitis B group according to the presence of abnormal liver function(alanine aminotransferase(ALT)≥>40 U/L,And n=45)and the carrier group(ALT 40 U/L,n=58),In addition,100 pregnant women with physical health during the same period were selected as the health group;To compare the levels of Lp-PLA2,Hcy and CRP between the three groups;The association of Lp-PLA2,Hcy,CRP and hepatitis B pregnancy,Subject operating characteristic curve(ROC)analyzed the value of Lp-PLA2,Hcy,and CRP in the diagnosis of hepatitis B pregnancy.Results:Lp-PLA2,Hcy,and CRP levels were significantly different between the hepatitis B,carrier,and healthy groups(P<0.05),Among them,hepatitis B group>carrier group>healthy group(P<0.05);The Logistic regression shows that,Lp-PLA2,Hcy,and CRP were all significantly associated with hepatitis B pregnancy,Is a predictive factor of hepatitis B pregnancy(P<0.05);The ROC analysis revealed that,The AUC of Lp-PLA2,Hcy,and CRP alone for all the diagnosis of hepatitis B pregnancy were 0.923,0.928 and 0.971,respectively,The AUC for the combined diagnosis of hepatitis B pregnancy was 0.972,Higher than for the individual indicators.Conclusion:Lp-PLA2,Hcy and CRP are closely related with hepatitis B pregnancy,and are significantly increased in hepatitis B pregnancy patients,which can be used as an auxiliary index for the diagnosis ofthis disease.
