摘 要:目的:探讨冠心病(CHD)患者血清组蛋白去乙酰化酶3(HDAC3)表达水平与冠状病变程度的关系。方法:选择2022年6月-2024年6月在本院接受冠状动脉造影术并诊断为CHD的患者110例(观察组),另外选择冠状动脉造影术无冠脉病变的健康人群42例(对照组),收集两组研究对象的一般临床资料及血清生化指标;根据Gensini评分将观察组患者分为轻度狭窄组(35例)、中度狭窄组(48例)及重度狭窄组(27例);ELISA法检测血清HDAC3表达水平;多因素Logistic回归分析影响CHD发生的因素;受试者工作特征(ROC)曲线分析血清HDAC3水平对CHD及CHD冠脉重度狭窄的预测价值。结果:观察组患者血清HDAC3、LDL-C水平高于对照组(P<0.05),血清HDAC3、LDL-C水平随着观察组患者冠脉狭窄程度的增加而升高,中度狭窄组、重度狭窄组血清HDAC3、LDL-C水平高于轻度狭窄组(P<0.05),重度狭窄组高于中度狭窄组(P<0.05);Gensini评分、高血压、糖尿病、血清LDL-C、HDAC3水平均是CHD发生的影响因素;血清HDAC3水平预测CHD及CHD冠脉病变狭窄程度的AUC分别为0.769、0.711;HDAC3、LDL-C联合预测CHD及其冠脉病变狭窄程度的AUC分别为0.875、0.865,高于单独检测(Z_(HDAC3-联合)=3.162,P=0.002,Z_(LDL-C-联合)=2.023,P=0.043;Z_(HDAC3-联合)=2120,P=0.034;Z_(LDL-C-联合)=2.332,P=0.019)。结论:冠心病患者血清HDAC3、LDL-C水平升高,与疾病严重程度有关,可能作为预测CHD及CHD冠脉病变程度的指标。Objective:To explore the relationship between the expression level of serum histone deacetylase 3(HDAC3)and the degree of coronary disease in patients with coronary heart disease(CHD).Methods:A total of 110 patients with CHD who underwent coronary angiography in our hospital from June 2022 to June 2024 were selected(observation group),in addition,42 healthy people without coronary artery disease after coronary angiography were selected(control group).The general clinical data and serum biochemical indexes of subjects in the two groups were collected.According to the Gensini score,the patients in the observation group were divided into the mild stenosis group(35 cases),moderate stenosis group(48 cases)and severe stenosis group(27 cases).The expression level of serum HDAC3 was detected by ELISA,and multi-factor Logistic regression was applied to analyze the risk factors affecting the occurrence of CHD.The predictive value of serum HDAC3 level on CHD and severe coronary stenosis in CHD was analyzed by ROC curve.Results:The level of serum HDAC3 in the observation group was higher than that in the control group(P<0.05).The serum HDAC3 level increased with the increase in the degree of coronary stenosis(P<0.05).Multivariate Logistic regression analysis showed that Gensini score,hypertension,diabetes,serum LDL-C,and HDAC3 levels were independent risk factors for CHD,and the AUC of serum HDAC3 level in predicting the degree of coronary stenosis in CHD and CHD was 0.769 and 0.711,respectively.The AUC of HDAC3 and LDL-C in predicting CHD and coronary stenosis was 0.875 and 0.865,respectively,and was higher than that of single detection(Z_(HDAC3-联合)=3.162,P=0.002,Z_(LDL-C-联合)=2.023,P=0.043;Z_(HDAC3-联合)=2120,P=0.034;Z_(LDL-C-联合)=2.332,P=0.019).Conclusion:The serum HDAC3,LDL-C level increases with the degree of coronary artery disease,which may be used as an index to predict the degree of coronary artery disease in CHD and CHD.
