摘　　要：目的:观察通督调神针法联合氟西汀治疗卒中后抑郁患者的效果。方法:选取2021年6月至2023年6月该院收治的114例卒中后抑郁患者进行前瞻性研究,根据随机数字表法将其分为对照组和研究组各57例。对照组采用盐酸氟西汀胶囊治疗,研究组在对照组基础上联合通督调神针法治疗,比较两组治疗前后抑郁[汉密尔顿抑郁量表(HAMD)]评分、日常生活能力[Barthel指数(BI)]评分、血清学指标[胰岛素样生长因子-1(IGF-1)、促肾上腺皮质激素释放激素(CRH)、促肾上腺皮质激素(ACTH)]水平和血清神经递质[5-羟色胺(5-HT)、多巴胺(DA)、脑源性神经营养因子(BDNF)、神经元特异性烯醇化酶(NSE)]水平。结果:治疗后,研究组HAMD评分低于对照组,BI评分高于对照组,差异均有统计学意义(P<0.05);治疗后,研究组IGF-1水平高于对照组,CRH、ACTH水平均低于对照组,差异有统计学意义(P<0.05);治疗后,研究组5-HT、DA、BDNF水平均高于对照组,NSE水平低于对照组,差异有统计学意义(P<0.05)。结论:通督调神针法联合氟西汀治疗卒中后抑郁患者可提高BI评分,改善血清学指标和血清神经递质水平,降低HAMD评分,效果优于单纯氟西汀治疗。Objective:To observe effects of Tongdu Tiaoshen acupuncture combined with Fluoxetine in treatment of patients with post-stroke depression.Methods:A prospective study was conducted on 114 patients with post-stroke depression admitted to the hospital from June 2021 to June 2023.According to the random number table method,they were divided into control group and study group,57 cases in each group.The control group was treated with Fluoxetine hydrochloride capsules,while the study group was treated with Tongdu Tiaoshen acupuncture on the basis of that of the control group.The depression[Hamilton depression scale(HAMD)]score,the activities of daily living[Barthel index(BI)]score,and the serological indexes[insulin-like growth factor-1(IGF-1),corticotropin-releasing hormone(CRH),adrenocorticotropic hormone(ACTH)]levels and serum neurotransmitters[5-hydroxytryptamine(5-HT),dopamine(DA),brain-derived neurotrophic factor(BDNF),neuron-specific enolase(NSE)]levels were compared between the two groups before and after the treatment.Results:After the treatment,the HAMD score of the observation group was lower than that of the control group,the BI score was higher than that of the control group,and the differences were statistically significant(P<0.05).After the treatment,the level of IGF-1 in the observation group was higher than that in the control group,the levels of CRH and ACTH were lower than those in the control group,and the differences were statistically significant(P<0.05).Further,after the treatment,the levels of 5-HT,DA and BDNF in the observation group were higher than those in the control group,the level of NSE was lower than that in the control group,and the differences were statistically significant(P<0.05).Conclusions:Tongdu Tiaoshen acupuncture combined with Fluoxetine in the treatment of the patients with post-stroke depression can improve the BI scores,improve the levels of serological indexes and serum neurotransmitter indexes,and reduce the HAMD scores.Moreover,it is superior to simple Fluoxetine hydrochl
