参附强心汤联合常规西药治疗慢性心力衰竭患者的效果

摘　　要：目的:观察参附强心汤联合常规西药治疗慢性心力衰竭患者的效果。方法:选取2021年10月至2023年10月该院收治的80例慢性心力衰竭患者进行前瞻性研究,按照随机数字表法将其分为研究组与对照组各40例。两组均予以对症治疗,在此基础上,对照组予以常规西药治疗,研究组在对照组基础上联合参附强心汤治疗,两组均治疗8周。比较两组临床疗效,治疗前后中医证候积分、心功能指标[左心室收缩末期容积指数(LVESVI)、左心室舒张末期容积指数(LVEDVI)、左心室射血分数(LVEF)、N末端B型利钠肽原(NT-proBNP)]水平、血管内皮功能指标[内皮素-1(ET-1)、一氧化氮(NO)]水平,以及不良反应发生率。结果:研究组治疗总有效率为95.00%(38/40),高于对照组的77.50%(31/40),差异有统计学意义(P<0.05);治疗后,两组口唇发绀、心悸气短、脘痞腹胀、面色无华、小便短少等中医证候积分均低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05);治疗后,两组LVESVI、LVEDVI、NT-proBNP水平均低于治疗前,且研究组低于对照组,两组LVEF水平均高于治疗前,且研究组高于对照组,差异有统计学意义(P<0.05);治疗后,两组ET-1水平均低于治疗前,且研究组低于对照组,两组NO水平均高于对照组,且研究组高于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:参附强心汤联合常规西药治疗慢性心力衰竭患者可提高治疗总有效率,降低中医证候积分,改善心功能指标、血管内皮功能指标水平,效果优于单纯常规西药治疗。Objective:To observe effects of Shenfu Qiangxin decoction combined with conventional Western medicine in treatment of patients with chronic heart failure.Methods:A prospective study was conducted on 80 patients with chronic heart failure admitted to the hospital from October 2021 to October 2023.According to the random number table method,they were divided into study group and control group,40 cases in each group.Both groups were given symptomatic treatment.On this basis,the control group was treated with conventional Western medicine,while the study group was treated with Shenfu Qiangxin decoction on the basis of that of the control group.Both groups were treated for 8 weeks.The clinical efficacies,the TCM syndrome scores,the levels of cardiac function indexes[left ventricular end-systolic volume index(LVESVI),left ventricular end-diastolic volume index(LVEDVI),left ventricular ejection fraction(LVEF),N-terminal pro-brain natriuretic peptide(NT-proBNP)]and vascular endothelial function indexes[endothelin-1(ET-1),nitric oxide(NO)]levels before and after the treatment,and the incidence of adverse reactions were compared between the two groups.Results:The total effective rate of treatment in the study group was 95.00%(38/40),which was higher than 77.50%(31/40)in the control group,and the difference was statistically significant(P<0.05).After the treatment,the scores of TCM syndromes such as cyanosis of lips,palpitation and shortness of breath,abdominal distension,pale complexion and oliguria in the two groups were lower than those before the treatment,those in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the levels of LVESVI,LVEDVI and NT-proBNP in the two groups were lower than those before the treatment,and those in the study group were lower than those in the control group;the LVEF levels of the two groups were higher than those before the treatment,and that in the study group was higher than that in the control group;an
