闭合复位钢板内固定术与传统切开复位内固定术治疗下肢复杂骨折患者的效果比较

摘　　要：目的:比较闭合复位钢板内固定术与传统切开复位内固定术治疗下肢复杂骨折患者的效果。方法:选取2020—2022年该院收治的100例下肢复杂骨折患者进行前瞻性研究,采用随机数字表法将其分为对照组与研究组各50例。对照组采用传统切开复位内固定术治疗,研究组采用闭合复位钢板内固定术治疗。比较两组手术相关指标(手术时间、手术切口长度、术中出血量)水平、术后疼痛程度[视觉模拟评分法(VAS)]评分、手术前后关节功能[美国纽约特种外科医院评分系统膝关节功能(HSS)]评分和并发症发生率。结果:两组手术时间比较,差异无统计学意义(P>0.05);研究组手术切口长度短于对照组,术中出血量少于对照组,差异均有统计学意义(P<0.05);术后7 d,两组VAS评分均低于术后1 d,且研究组低于对照组,差异有统计学意义(P<0.05);术后3、6个月,两组HSS评分均高于术前,且研究组高于对照组,差异有统计学意义(P<0.05);研究组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:闭合复位钢板内固定术治疗下肢复杂骨折患者可提高HSS评分,缩短手术切口长度,减少术中出血量,以及降低VAS评分和并发症发生率的效果优于传统切开复位内固定术治疗。Objective:To compare effects of closed reduction plate internal fixation and conventional open reduction internal fixation in treatment of patients with complex fractures of lower limbs.Methods:A prospective study was conducted on 100 patients with complex fractures of the lower extremities admitted to the hospital from 2020 to 2022.They were divided into control group and study group by using the random number table method,50 cases in each group.The control group was treated with conventional open reduction and internal fixation,while the study group was treated with closed reduction and plate internal fixation.The operation related indexes(operation time,incision length,intraoperative blood loss)levels,the postoperative pain degree[visual analogue scale(VAS)]score and the joint function[Hospital for Special Surgery knee score(HSS)]score before and after the surgery,and the incidence of complications were compared between the two groups.Results:There was no significant difference in the operation time between the two groups(P>0.05).The incision length of the study group was shorter than that of the control group,the intraoperative blood loss was less than that of the control group,and the differences were statistically significant(P<0.05).At 7 days after the surgery,the VAS scores of the two groups were lower than those 1 day after the surgery,that in the study group was lower than that in the control group,and the differences were statistically significant(P<0.05).At 3 and 6 months after the surgery,the HSS scores of the two groups were higher than those before the surgery,those in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).Further,the incidence of complications in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusions:Closed reduction and plate internal fixation in the treatment of the patients with complex fractures of lower limbs can improve the HSS scores,shorten the
