基于远程家庭护理计划的健康管理在老年2型糖尿病患者中的应用效果

摘　　要：目的:观察基于远程家庭护理计划的健康管理在老年2型糖尿病患者中的应用效果。方法:选取2022年1—12月该院收治的112例老年2型糖尿病患者进行前瞻性研究,采用随机数字表法将其分为对照组(n=56)和观察组(n=56)。对照组采用常规健康管理,观察组采用基于远程家庭护理计划的健康管理,比较两组管理前后血糖达标率、遵医行为优良率,管理前后自我效能[自我效能感量表(GSES)]、自护能力[自护能力测定量表(ESCA)]评分,以及护理期间并发症发生率。结果:观察组血糖达标率为92.86%(52/56),高于对照组的78.57%(44/56),差异有统计学意义(P<0.05);观察组遵医行为优良率为91.07%(51/56),高于对照组的76.79%(43/56),差异有统计学意义(P<0.05);管理后,两组GSES、ESCA评分均高于管理前,且观察组高于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为10.71%(6/56),低于对照组的33.93%(19/56),差异有统计学意义(P<0.05)。结论:基于远程家庭护理计划的健康管理应用于老年2型糖尿病患者可提高血糖达标率、遵医行为优良率、GSES评分和ESCA评分,降低并发症发生率,效果优于常规健康管理。Objective:To observe application effects of health management based on remote home care plan in elderly patients with type 2 diabetes.Methods:A prospective study was conducted on 112 elderly patients with type 2 diabetes admitted to this hospital from January to December 2022.They were divided into control group(n=56)and observation group(n=56)by using the random number table method.The control group was treated with conventional health management,while the observation group was treated with the health management based on remote home care plan.The blood glucose compliance rate,the excellent and good rate of compliance behavior,the self-efficacy[self-efficacy scale(GSES)]and self-care ability[self-care ability measurement scale(ESCA)]scores before and after the management,and the incidence of complications during the nursing were compared between the two groups.Results:The blood glucose compliance rate of the observation group was 92.86%(52/56),which was higher than 78.57%(44/56)of the control group,and the difference was statistically significant(P<0.05).The excellent and good rate of compliance behavior in the observation group was 91.07%(51/56),which was higher than 76.79%(43/56)in the control group,and the difference was statistically significant(P<0.05).After the management,the scores of GSES and ESCA in the two groups were higher than those before the management,those in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Further,the incidence of complications in the observation group was 10.71%(6/56),which was lower than 33.93%(19/56)in the control group,and the difference was statistically significant(P<0.05).Conclusions:The health management based on remote home care plan in the elderly patients with type 2 diabetes can improve the blood glucose compliance rate,the excellent and good rate of compliance behavior,the GSES scores and the ESCA scores,and reduce the incidence of complications.Moreover,it is superior to the conventional he
