摘　　要：目的探讨三酰甘油-葡萄糖(TyG)指数、TyG-体质量指数(BMI)和TyG-腰高比(WHtR)与中国老年人肌肉减少症之间的关系。方法根据中国健康和老龄化纵向研究(CHARLS)数据,检测TyG、TyG-BMI和TyG-WHtR指数水平。采用多因素调整后的Logistic回归分析、限制性立方样条图(RCS)回归模型、交互分层分析来探讨TyG、TyG-BMI和TyG-WHtR指数与肌肉减少症患病风险之间的关系。结果本研究共纳入2355名参与者,中位年龄65.0(62.0,70.0)岁。经多变量调整后,TyG指数[OR 95%CI:0.52(0.44,0.63),P<0.01]、TyG-BMI指数[OR 95%CI:0.95(0.95,0.96),P<0.01]、TyG-WHtR指数[OR 95%CI:0.99(0.99,0.99),P<0.01]均与肌肉减少症患病风险相关。RCS回归模型结果显示TyG、TyG-BMI和TyG-WHtR指数与肌肉减少症之间均存在非线性相关性(非线性P<0.05)。糖尿病药物治疗在TyG、TyG-WHtR指数与肌肉减少症患病风险的关系中起交互作用(P_(交互)<0.05)。结论TyG、TyG-BMI和TyG-WHtR指数与肌肉减少症的风险增加相关,可以作为评估中国老年人群肌肉减少症风险的有效指标。Objective To investigate the association between triglyceride-glucose(TyG)index,TyG-body mass index(BMI),and TyG-waist-to-height ratio(WHtR)with the risk of sarcopenia in the Chinese elderly population.Methods The levels of TyG,TyG-BMI and TyG-WHtR were examined based on data from the China Health and Retirement Longitudinal Study(CHARLS).Multivariable-adjusted logistic regression,restricted cubic spline(RCS)regression models,and stratified analysis with interaction were utilized to analyze the association between TyG,TyG-BMI,TyG-WHtR with the risk of sarcopenia.Results A total of 2355 participants with a medi-an age of 65.0(62.0,70.0)years were included in the study.The TyG(OR,95%CI:0.52,0.44-0.63,P<0.01),TyG-BMI(OR,95%CI:0.95,0.95-0.96,P<0.01),TyG-WHtR(OR,95%CI:0.99,0.99-0.99,P<0.01)were all associated with the risk of sarcopenia after multivariate adjustment.The RCS regression model showed a non-linear correlation between TyG,TyG-BMI,TyG-WHtR and the risk of sarcopenia(nonlinear P<0.05).Further subgroup analysis demonstrated that diabetes medication played a significant interaction in the association between TyG,TyG-WHtR indexes and the risk of sarcopenia(P for interaction<0.05).Conclusion TyG,TyG-BMI and TyG-WHtR are as-sociated with an increased risk of sarcopenia and can be used as valid indicators to assess the risk of sarcopenia in the Chinese elderly population.
