摘 要:目的:分析腰椎间盘突出症患者应用体外冲击波联合腰部核心肌力训练对腰椎功能、腰椎活动度及炎症因子水平的影响。方法:选取河南电力医院2021年10月至2023年10月收治的106例腰椎间盘突出症患者为研究对象,以随机数表法分组。基础组53例给予腰部核心肌力训练,研究组53例在此基础上增加体外冲击波。对比两组患者腰椎功能、腰椎活动度、炎症因子水平。结果:干预后,两组日常活动受限度、主观症状、临床体征分值均降低,但研究组降低幅度较基础组高(P<0.05);干预后,两组腰椎旋转活动度、侧屈活动度、屈伸活动度均升高,但研究组升高幅度较基础组高(P<0.05);干预后,两组基质金属蛋白酶3(matrix metalloproteinase 3,MMP-3)、白细胞介素-1β(interleukin-1,IL-1β)、白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-alpha,TNF-α)水平均降低升高,但研究组降低幅度较基础组高(P<0.05)。结论:体外冲击波、腰部核心肌力训练联合应用能够有效改善腰椎间盘突出症患者腰椎功能,提高腰椎活动度,减轻炎症反应。Objective:To analyze the impact of extracorporeal shock wave therapy combined with lumbar core muscle training on lumbar function,lumbar spine mobility and inflammatory factor levels in patients with lumbar disc herniation.Methods:106 patients with lumbar disc herniation from October 2021 to October 2023 at Henan Electric Power Hospital were selected and randomized into two groups using a random number table.The basic group of 53 patients received lumbar core muscle training,while the study group of 53 patients received extracorporeal shock wave therapy in addition to lumbar core muscle training.Lumbar function,lumbar spine mobility and inflammatory factor levels were compared between the two groups.Results:After intervention,scores for daily activity limitation,subjective symptoms and clinical signs decreased in both groups,but the reduction was more significant in the study group compared to the basic group(P<0.05).Post-intervention,lumbar rotation,lateral bending and flexion-extension range of motion increased in both groups,but the increase was more pronounced in the study group(P<0.05).Following intervention,the levels of Matrix metalloproteinase 3(MMP-3),interleukin-1β(IL-1β),interleukin-6(IL-6)and tumor necrosis factor-alpha(TNF-α)decreased in both groups,but the decrease was greater in the study group compared to the basic group(P<0.05).Conclusion:The combined application of extracorporeal shock wave therapy and lumbar core muscle training can effectively improve lumbar function,enhance lumbar spine mobility,and reduce inflammatory responses in patients with lumbar disc herniation.
