腰椎间盘突出症术后椎间隙感染患者心理健康及生活质量分析

摘　　要：目的:探讨腰椎间盘突出症(LDH)术后椎间隙感染患者的心理健康状况及生活质量的相关影响因素。方法:选取2020年1月至2023年12月于驻马店市中心医院脊柱外科进行治疗的LDH术后椎间隙感染患者82例进行回顾性分析,采用医院焦虑抑郁量表(HADS)评估患者心理健康,采用生活质量指数表(QL-Index)评估患者生活质量,通过调查问卷采集患者的基础资料,对基础资料进行单因素分析后,获得的显著因素进行多因素Logistic回归分析和Spearman相关性分析,分析LDH术后椎间隙感染患者心理健康及生活质量的影响因素。结果:82例患者中有48例心理健康良好,34例心理不健康;52例生活质量好,30例生活质量差。单因素分析发现LDH术后椎间隙感染患者的心理健康与文化程度、家庭经济情况、吸烟史、锻炼情况、基础疾病史有关(P<0.05),生活质量与文化程度、吸烟史、基础疾病史有关(P<0.05);Logistic多因素分析显示,文化程度、家庭经济情况、吸烟史、基础疾病史是LDH术后椎间隙感染患者的心理健康的独立危险因素(P<0.05),文化程度、吸烟史、基础疾病史是生活质量的独立危险因素(P<0.05),Spearman相关性分析发现LDH术后椎间隙感染患者的心理健康与文化程度、家庭经济情况呈现显著的正相关性(P<0.05),与吸烟史、基础疾病史呈现显著的负相关性(P<0.05)。生活质量与文化程度呈现显著的正相关性(P<0.05)与吸烟史呈现显著的负相关性(P<0.05)。结论:LDH术后椎间隙感染患者存在心理健康欠缺及生活质量低下的风险,与文化程度呈现显著的正相关性、与吸烟史呈现显著的负相关性,在LDH术后椎间隙感染患者防治工作中应关注患者心理健康及生活质量。Objective:To explore the mental health status and related influencing factors of quality of life in patients with postoperative intervertebral disc space infection following lumbar disc herniation(LDH)surgery.Methods:A retrospective analysis was conducted on 82 patients with postoperative intervertebral disc space infection following LDH surgery who were treated in the Spine Surgery Department of Zhumadian Central Hospital from January 2020 to December 2023.The Hospital Anxiety and Depression Scale(HADS)was used to assess patients'mental health,and the Quality of Life Index(QL-Index)was used to evaluate their quality of life.Basic information was collected through questionnaires.After univariate analysis of the basic information,significant factors were subjected to multivariate Logistic regression analysis and Spearman correlation analysis to identify the influencing factors of mental health and quality of life in patients with postoperative intervertebral disc space infection following LDH surgery.Results:Among the 82 patients,48 had good mental health and 34 had poor mental health;52 had good quality of life and 30 had poor quality of life.Univariate analysis found that mental health in patients with postoperative intervertebral disc space infection following LDH surgery was related to education level,family economic status,smoking history,exercise habits and history of underlying diseases(P<0.05).Quality of life was related to education level,smoking history and history of underlying diseases(P<0.05).Multivariate Logistic analysis showed that education level,family economic status,smoking history and history of underlying diseases were independent risk factors for mental health in patients with postoperative intervertebral disc space infection following LDH surgery(P<0.05).Education level,smoking history and history of underlying diseases were independent risk factors for quality of life(P<0.05).Spearman correlation analysis found that mental health in patients with postoperative intervertebral disc space inf
