UBE技术在合并基础疾病的老年腰椎椎管狭窄症中的临床应用

摘　　要：目的:探讨单侧双通道脊柱内镜(unilateral biportal endoscopy,UBE)技术治疗合并基础疾病的老年腰椎椎管狭窄症的临床效果及安全性。方法:分析辽宁省人民医院骨五科在2022年1月至2022年10月经单侧双通道脊柱内镜(UBE)技术治疗的20例老年腰椎椎管狭窄症患者,均经严格保守治疗3个月无效患者。手术节段涉及L_(3)/L_(4)、L_(4)/L_(5)、L_(5)/S_(1)。手术方式采用UBE技术在关节镜下进行减压,比较术前、术后ODI、VAS评分。结果:20例患者手术均顺利,术中发生2例硬膜撕裂,未出现切口感染、神经损伤等并发症;VAS和ODI评分术后均低于术前(P<0.05)。结论:单侧双通道脊柱内镜(UBE)技术治疗合并基础疾病的老年腰椎椎管狭窄症患者,并发症少,术后腰痛轻、恢复快,可获良好效果,相信UBE技术在老年腰椎管狭窄症中具有广阔的临床应用前景。Objective:To investigate the clinical efficacy and safety of the Unilateral Biportal Endoscopy(UBE)technique in the treatment of lumbar spinal stenosis in elderly patients accompanied by underlying diseases.Methods:20 elderly patients with lumbar spinal stenosis who underwent UBE surgery in the Department of Orthopedics 5 of Liaoning Provincial People's Hospital between January 2022 and October 2022 were analyzed.All patients had undergone strict conservative treatment for 3 months without improvement.The surgical segments involved L_(3)/L_(4),L_(4)/L_(5) and L_(5)/S_(1).The surgical approach involved decompression using the UBE technique under arthroscopy.Preoperative and postoperative ODI and VAS scores were compared.Results:All 20 patients underwent successful surgery.During the operation,there were 2 cases of dural tears,but no complications such as wound infection or nerve injury occurred.Both VAS and ODI scores were lower postoperatively compared to preoperatively(P<0.05).Conclusion:The treatment of elderly patients with lumbar spinal stenosis accompanied by underlying diseases using UBE technology results in fewer complications,mild postoperative low back pain,and rapid recovery,leading to favorable outcomes.UBE technology holds promising clinical application prospects in the treatment of lumbar spinal stenosis among the elderly patients.
