摘　　要：溃疡性结肠炎(ulcerative colitis,UC)是一种慢性非特异性肠道炎症性疾病,其发病机制复杂,涉及遗传、环境、免疫等多因素相互作用。近年来,UC的发病率在全球范围内逐年上升。在中医学中,肾为“先天之本”,脾为“后天之本”,脾肾二脏在UC的发生、发展中起着至关重要的作用。脾肾亏损已被证实是UC的重要病机之一。中医的“后天之本”理论为理解UC的病理生理提供了独特的视角,并认为脾肾功能失调与UC的发生和发展密切相关,从脾肾二脏入手,深入探究肠道微生物群失衡和免疫系统异常之间的相互作用及其在UC发病中的具体机制。基于“后天之本”理论的诊治策略,旨在通过调理脾肾功能来恢复肠道微生态平衡和免疫系统的正常功能。以温补脾肾法来治疗UC。并附验案1则。Ulcerative Colitis(UC)is a chronic,nonspecific inflammatory disease of the intestines with complex pathogenesis involving multiple factors such as genetics,environment,and immunity.Globally,the incidence of UC has been increasing,and with societal advancements,its prevalence has notably risen in China.In traditional Chinese medicine,the kidney is considered the"foundation of prenatal essence,"and the spleen is the"foundation of postnatal nutrition."Both play a crucial role in the occurrence and progression of UC.Spleen-kidney deficiency has been confirmed as one of the key pathomechanisms of UC.The theory of primordial essence'in traditional Chinese medicine provides a unique perspective for understanding the pathophysiology of UC and suggests that dysfunction of the spleen and kidney is closely related to the onset and development of UC.By focusing on the spleen and kidney,we can delve into the interactions between intestinal microbiota imbalance and immune system abnormalities,as well as their specific mechanisms in the pathogenesis of UC.The therapeutic strategy based on the primordial essence'theory aims to restore intestinal microbial balance and normal immune function through the regulation of spleen-kidney function.Warm supplementation therapy for the spleen and kidney is used to treat UC,supported by a case study.
