正交试验下电针悬钟穴治疗脑梗死后足下垂的优化研究

摘　　要：目的通过比较电针悬钟穴不同频率、针刺深度、留针时间对脑梗死后足下垂患者的疗效差异,筛选出最优化的方案。方法选取80例脑梗死后足下垂患者为研究对象,按照3因素2水平的正交试验设计方法随机分为8组,每组各10例。对8组患者分别采用不同频率、针刺深度、留针时间进行电针悬钟穴治疗,1次/d,连续治疗2周。于治疗结束后评估疗效,并总结采用电针悬钟穴对脑梗死后足下垂患者治疗的最优方案。结果刺激频率是影响电针悬钟穴治疗脑梗死后足下垂患者疗效的主要因素,其次为针刺深浅和留针时间。电针悬钟穴治疗脑梗死后足下垂患者最优组合为针刺1.5寸+频率100 Hz+留针20 min或40 min。结论对于脑梗死后足下垂患者采用电针悬钟穴进行治疗能够改善MAS评分、GMFM评分及简化Fugl-Meyer运动功能评分。刺激频率、针刺深度、留针时间影响电针悬钟穴治疗脑梗死后足下垂患者疗效的主次关系为:刺激频率>针刺深浅>留针时间,其中针刺1.5寸+频率100 Hz+留针20 min或40 min为优选方案。Objective The optimal scheme was screened by comparing the therapeutic effects of electroacupuncture at Xuanzhong Point with different frequencies,acupuncture depths,and needle retention time on foot drop after cerebral infarction.Methods Eighty patients with foot drop after cerebral infarction were selected as the research objects,and were randomly divided into 8 groups according to the orthogonal experimental design of 3 factors and 2 levels,with 10 cases in each group.The patients in the 8 groups were treated with electroacupuncture at Xuanzhong Point with different frequencies,acupuncture depth,and needle retention time,once a day for 2 weeks.At the end of treatment,the therapeutic effect was evaluated,and the optimal scheme of electroacupuncture at the Xuanzhong Point for foot drop after cerebral infarction was summarized.Results The frequency of stimulation was the main factor affecting the therapeutic effect of electroacupuncture at the Xuanzhong acupoint on patients with foot drop after cerebral infarction,followed by the depth of acupuncture and the duration of needle retention.The optimal plan of electroacupuncture at Xuanzhong acupoint for treating patients with foot drop after cerebral infarction was acupuncture at 1.5 cun+frequency of 100 Hz,and needle retention for 20 min or 40 min.Conclusion The primary and secondary relationship of stimulation frequency,acupuncture depth,and needle retention time affecting the therapeutic effect of electroacupuncture at Xuanzhong Point in patients with foot drop after cerebral infarction is stimulation frequency>acupuncture depth>needle retention time,among which 1.5 cun+frequency 100Hz+needle retention 20min or 40 min is the optimal scheme.
