阿奇霉素联合中药汤剂治疗儿童肺炎支原体肺炎的疗效

摘　　要：目的探究阿奇霉素联合中药汤剂治疗儿童肺炎支原体肺炎(Mycoplasmal pneumoniae pneumonia,MPP)(痰热闭肺证)的疗效。方法选取2021年12月至2023年1月收治的94例MMP患儿作为研究对象,用随机数字表法分为为观察组和对照组,每组47例。对照组在对症治疗的基础上使用阿奇霉素干混悬剂治疗,观察组在对照组治疗的基础上加用中药汤剂治疗。比较2组的疗效,治疗前后的中医证候积分,血清粒细胞集落刺激因子(granulocyte colony-stimulating factor,G-CSF)水平,症状消失时间,抗体免疫球蛋白M(immunoglobulin M,IgM)转阴率和不良反应的发生情况。结果与对照组(80.85%,38/47)比较,观察组的治疗总有效率(95.75%,45/47)显著升高(χ^(2)=5.045,P=0.025);治疗后,2组的中医证候积分、血清G-CSF水平均显著降低(P<0.05),且观察组的中医证候积分、血清G-CSF水平均显著低于对照组(P<0.05);与对照组比较,观察组的咳嗽、发热和肺部啰音消失时间均显著缩短(P<0.05);与对照组(74.47%,35/47)比较,观察组治疗后的抗体IgM转阴率(93.62%,44/47)显著升高(χ^(2)=6.425,P=0.011);2组不良反应发生率比较差异无统计学意义(χ^(2)=0.927,P=0.336)。结论阿奇霉素联合中药汤剂治疗儿童MPP的效果显著,有助于缓解症状,提高IgM的转阴率,降低血清G-CSF水平,且安全性较好。Objective To investigate the efficacy of azithromycin combined with traditional Chinese medicine decoction in the treatment of Mycoplasmal pneumoniae pneumonia(MPP)in children(phlegm-heat closed lung syndrome).Methods A total of 94 children with MMP who were admitted from December 2021 to January 2023 were selected as the study subjects,and they were divided into observation group and control group,with 47 cases in each group.The control group was treated with azithromycin dry suspension on the basis of symptomatic treatment,and the observation group was treated with traditional Chinese medicine(TCM)decoction on the basis of treatment in the control group.The efficacy,TCM pattern score,serum granulocyte colony-stimulating factor(G-CSF)level,symptom disappearance time,antibody immunoglobulin M(IgM)conversion rate and adverse reactions before and after treatment were compared between the 2 groups.Results Compared with the control group(80.85%,38/47),the total effective rate of treatment in the observation group(95.75%,45/47)was significantly increased(χ^(2)=5.045,P=0.025).After treatment,the TCM pattern score and serum G-CSF level of the 2 groups were significantly decreased(P<0.05),and the TCM pattern score and serum G-CSF level of the observation group were significantly lower than those of the control group(P<0.05),compared with the control group,the disappearance time of cough,fever and pulmonary rales in the observation group was significantly shortened(P<0.05),and compared with the control group(74.47%,35/47),the antibody IgM in the observation group after treatment was significantly shortened,IgM was significantly increased(93.62%,44/47)(χ^(2)=6.425,P=0.011).There was no significant difference in the incidence of adverse reactions between the 2 groups(χ^(2)=0.927,P=0.336).Conclusion Azithromycin combined with traditional Chinese medicine decoction has a significant effect in the treatment of MPP in children,which helps to alleviate symptoms,improve the negative conversion rate of IgM,reduce serum G-CSF le
