摘　　要：目的研究秦艽羌活汤加减辅助改善肛肠病术后湿热下注证患者肛门瘙痒症的效果及对血清超敏C反应蛋白(high sensitivity C-reactive protein,hs-CRP)、淀粉样蛋白A(serum amyloid A protein,SAA)及中性粒细胞与淋巴细胞比值(neutrophil to lymphocyte ratio,NLR)水平的影响。方法用随机数字表法将2022年5月至2023年10月收治的92例肛肠病术后出现肛门瘙痒症(湿热下注证)的患者分为中医组和常规组,各46例。常规组术后采用糠酸莫米松乳膏进行治疗,中医组给予秦艽羌活汤加减配合糠酸莫米松乳膏治疗,2组均治疗1周,治疗后评估2组的临床疗效,并比较2组治疗前后的中医证候积分、炎症指标、生活质量及不良反应的发生情况。结果中医组的治疗总有效率为95.65%,高于常规组的82.61%(P<0.05)。治疗前2组患者的中医证候积分,hs-CRP、SAA、NLR水平及皮肤病生活质量指标量表(dermatology life quality index,DLQI)各维度得分比较差异均无统计学意义(P>0.05);治疗后,中医组患者的中医证候积分,hs-CRP、SAA、NLR水平及DLQI量表各维度得分均低于常规组(P<0.05);2组患者不良反应发生率比较差异无统计学意义(P>0.05)。结论秦艽羌活汤加减辅助能够有效改善患者的肛门瘙痒,缓解炎症反应,提高患者的生活质量,且安全性较好,具有临床推广价值。Objective To study the effect of Modified Qinjiao Qianghuo Decoction on the improvement of damp-heat pouring downward syndrome of anal pruritus after anorectal surgery as well as its impact on serum levels of high sensitivity C-reactive protein(hs-CRP),serum amyloid A protein(SAA)and neutrophil to lymphocyte ratio(NLR).Methods Utilizing the random number table method,92 patients with anal pruritus(damp-heat pouring downward syndrome)after anorectal surgery in the hospital from May 2022 to October 2023 were divided into the traditional Chinese medicine(TCM)group and the conventional group,with 46 cases in each group.The conventional group received treatment with Mometasone Furoate Cream post-surgery,while the TCM group received Modified Qinjiao Qianghuo Decoction in conjunction with Mometasone Furoate Cream.Both groups were treated for 1 week.The clinical efficacy was assessed after treatment,and comparisons were made regarding TCM syndromes scores,inflammatory markers,quality of life,and adverse reactions before and after treatment.Results The total effective treatment rate in the TCM group was 95.65%,higher than the 82.61%observed in the conventional group(P<0.05).Prior to treatment,TCM syndromes scores,the serum levels of hs-CRP,SAA and NLR,and dermatology life quality index(DLQI)scores were comparable between the 2 groups(P>0.05).Post-treatment,these scores and levels in the TCM group were lower than those in the conventional group(P<0.05).The incidence of adverse reactions showed no significant difference between the 2 groups(P>0.05).Conclusion Modified Qinjiao Qianghuo Decoction is effective in improving anal pruritus,mitigating inflammatory responses,and enhancing quality of life,demonstrating potential for clinical application.
