复方多黏菌素B软膏联合裸花紫珠口服对肛周脓肿患者术后创面愈合及血管内皮因子的影响

摘　　要：目的探究复方多黏菌素B软膏联合裸花紫珠口服对肛周脓肿患者术后创面愈合及血管内皮因子的影响。方法回顾性分析2022年5月至2023年12月西部战区空军医院收治的138例肛周脓肿患者的资料,患者均择期行切开引流术治疗,根据患者术后创面治疗方案将其分为常规组和观察组。常规组59例,采用术后常规创面管理,观察组79例,给予复方多黏菌素B软膏联合裸花紫珠口服治疗。比较2组术后创面愈合情况及干预前后血清相关生长因子水平。结果观察组的临床总有效率为92.41%,高于常规组的79.66%(P<0.05);观察组的创面愈合时间短于常规组[(16.15±1.88)d vs.(20.23±2.59)d,t=10.725,P<0.05];干预后观察组的创面疼痛视觉模拟评分(visual analogue score,VAS)、创面分泌物评分、肉芽生长评分分别为(1.56±0.31)、(0.76±0.14)、(1.02±0.34)分,低于常规组的(3.12±0.84)、(1.15±0.32)、(1.89±0.51)分,差异具有统计学意义(t=15.194、9.672、12.010,P<0.05);2组干预后的血清血管内皮生长因子(vascular endothelial growth factor,VEGF)、血小板衍化内皮细胞生长因子(plateletderived endothelial cell growth factor,PDGF)、成纤维细胞生长因子(fibroblast growth factor,FGF)-2均上升,观察组干预后上述因子水平分别为(61.59±6.13)、(85.19±4.27)、(102.58±9.44)ng·L^(−1),高于常规组的(56.44±6.05)、(78.18±3.52)、(84.65±8.46)ng·L^(−1),差异具有统计学意义(t=4.910、10.268、11.533,P<0.05)。结论复方多黏菌素B软膏联合裸花紫珠口服对肛周脓肿患者术后创面愈合的疗效确切,可以缩短创面愈合时间,减轻疼痛,提高生长因子水平,促进创面愈合。Objective To investigate the effects of Compound Polymyxin B Ointment combined with oral administration of Luohuazizhu(Callicarpa nudiflora Hook.et Arn)on wound healing and vascular endothelial factors after perianal abscess surgery.Methods 138 patients with perianal abscess admitted to Air Force Hospital of the Western Theater Command from May 2022 to December 2023 were retrospectively analyzed.All patients were treated with incision and drainage at selected time.According to the postoperative wound treatment plan,they were divided into conventional group and observation group.The conventional group of 59 patients received conventional wound management after surgery,and the observation group of 79 patients received Compound Polymyxin B Ointment combined with oral treatment of Luohuazizhu.Postoperative wound healing and serum related growth factor levels before and after intervention were compared between the 2 groups.Results The total effective rate of observation group was 92.41%,higher than that of conventional group(79.66%)(P<0.05).The wound healing time of observation group was shorter than that of conventional group[(16.15±1.88)d vs.(20.23±2.59)d,t=10.725,P<0.05].After intervention,the visual analogue score,wound secretion score and granulation growth score in observation group were(1.56±0.31),(0.76±0.14)and(1.02±0.34)points respectively,which were lower than those in conventional group(3.12±0.84),(1.15±0.32)and(1.89±0.51)points,with the difference is statistically significant(t=15.194,9.672,12.010,P<0.05).The serum levels of vascular endothelial growth factor(VEGF),platelet-derived endothelial cell growth factor(PDGF)and fibroblast growth factor(FGF)-2 were increased in both groups after intervention,and the levels of above factors in observation group were(61.59±6.13),(85.19±4.27)and(102.58±9.44)ng·L^(−1),respectively,higher than those in the conventional group(56.44±6.05),(78.18±3.52)and(84.65±8.46)ng·L^(−1),and the differences were statistically significant(t=4.910,10.268,11.533,P<0.
