左甲状腺素钠片联合硒酵母片对甲状腺癌患者术后血清T淋巴细胞亚群和细胞因子的影响

摘　　要：目的探究左甲状腺素钠片联合硒酵母片对甲状腺癌患者术后血清T淋巴细胞亚群和相关细胞因子的影响。方法回顾性分析118例河北省第一荣军优抚医院2021年2月至2024年3月收治的甲状腺癌患者的资料,均择期进行甲状腺癌切除术,根据术后不同药物治疗方案分为对照组(55例,给予左甲状腺素钠片治疗)和观察组(63例,给予左甲状腺素片联合硒酵母片治疗)。比较2组的临床疗效、治疗前后的血清T淋巴细胞亚群[T细胞抗原分化簇3受体(differentiation cluster 3^(+),CD3^(+))、T细胞抗原分化簇4受体(differentiation cluster 4^(+),CD4^(+))、T细胞抗原分化簇8受体(differentiation cluster8^(+),CD8^(+))]、细胞因子[白细胞介素-6(interleukin,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、血管内皮细胞生长因子(vascular endothelial growth factor,VEGF)]、促甲状腺素(thyroid stimulating hormone,TSH)、甲状腺球蛋白(thyroglobulin,Tg)和甲状腺球蛋白抗体(thyroglobulin antibodies,TgAb)水平以及不良反应事件的发生情况等。结果观察组的临床总有效率为95.24%,高于对照组的83.64%(P<0.05);与治疗前比较,2组治疗后的CD3^(+)、CD4^(+)、FT_(3)、FT_(4)水平均升高,且观察组治疗后上述指标均高于对照组(P<0.05);治疗后2组的CD8^(+)、IL-6、TNF-α、VEGF、TSH、Tg、TgAb水平较治疗前均下降,且观察组上述各指标的水平较对照组更低(P<0.05);2组药物不良反应发生率比较,差异无统计学意义(P>0.05)。结论左甲状腺素钠片联合硒酵母片对甲状腺癌术后患者治疗效果确切,可以增强患者的细胞免疫功能,改善甲状腺功能,提升临床疗效,且安全性良好。Objective To explore the effects of Levothyroxine Sodium Tablets combined with Selenium Yeast Tablets on serum T lymphocyte subsets and related cytokines in patients with thyroid cancer after surgery.Methods 118 patients with thyroid cancer admitted to the First Veterans’Welfare Hospital of Hebei Province from February 2021 to March 2024 were retrospectively analyzed.All patients underwent elective thyroid cancer resection.According to the postoperative drug treatment plan,they were divided into a reference group(55 cases,receiving the treatment with Levothyroxine Sodium Tablets)and a observation group(63 cases,receiving the treatment with Levothyroxine Tablets combined with Selenium Yeast Tablets).The clinical efficacy,the levels of serum T lymphocyte subsets[T cell antigen differentiation clusterreceptors(CD3^(+),CD4^(+)and CD8^(+))],cytokines[interleukin-6(IL-6),tumor necrosis factor(TNF-α),and vascular endothelial growth factor(VEGF)],thyroid stimulating hormone(TSH),thyroglobulin(Tg)and thyroglobulin antibodies(TgAb),and adverse events were compared between the 2 groups before and after treatment.Results The total clinical effective rate of the observation group was 95.24%,higher than 83.64%of the reference group(P<0.05).Compared with before treatment,the levels of CD3^(+),CD4^(+),FT_(3)and FT_(4)in both groups were increased after treatment,and the above indexes in the observation group were higher than those in the reference group after treatment(P<0.05).After treatment,the levels of CD8^(+),IL-6,TNF-α,VEGF,TSH,Tg and TgAb in the 2 groups were decreased compared with those before treatment,and the levels of the above indexes in the observation group were lower than those in the reference group(P<0.05).There was no significant difference in the incidence of adverse drug reactions between the 2 groups(P>0.05).Conclusion Levothyroxine Sodium Tablets combined with Selenium Yeast Tablets have a definite therapeutic effect in postoperative thyroid cancer patients,which can enhance cellular immune function,im
