老年人血清肌酐/胱抑素C比值和躯体功能的相关性研究

摘　　要：目的探究社区老年人群血清肌酐/胱抑素C比值和躯体功能的相关性。方法选择在社区进行年度体检的老年人203例为研究对象进行横断面调查,所有老年人完成简易体能测量表(SPPB)评定,并根据SPPB评分结果分为躯体功能减退组(0~9分)和躯体功能正常组(10~12分)。比较2组间一般资料的差异。采用Spearman相关分析和二元logistic回归分析探讨血清肌酐/胱抑素C比值和躯体功能的相关性。结果躯体功能减退组的年龄、5次起坐时间、Hb、总胆红素、尿素氮和胱抑素C水平均高于躯体功能正常组(P<0.05)。躯体功能正常组的握力、步速、四肢骨骼肌指数(ASMI)、维生素D水平、血清肌酐/胱抑素C比值、游离三碘甲状腺原氨酸水平均高于躯体功能减退组(P<0.05)。Spearman相关分析显示,血清肌酐/胱抑素C比值与握力、步速、ASMI、SPPB评分呈正相关(P<0.01),与5次起坐时间呈负相关(P<0.01)。二元logistic回归分析结果显示,血清肌酐/胱抑素C比值是躯体功能的独立影响因素。结论在社区老年人群血清肌酐/胱抑素C比值与躯体功能减退关系密切,可能是识别躯体功能下降的简易血清学指标。Objective To investigate the correlation between serum creatinine-to-cystatin C ratio and physical performance in the community elderly.Methods A total of 203 cases of elderly people who underwent physical examination in the community were enrolled in this cross-sectional study.All the elderly underwent the assessment of short physical performance battery(SPPB)scale,and they were divided into poor physical function group(0-9 points)and good physical function group(10-12 points)based on the SPPB score.The general data were compared between the two groups.Spearman correlation analysis and binary logistic regression analysis were used to analyze the relationship between serum creatinine-to-cystatin C ratio and physical performance.Results The levels of age,5-time chair stand test,hemoglobin,bilirubin,urea nitrogen and cystatin C were higher,and the levels of grip strength,pace,appendicular skeletal muscle index(ASMI)score,vitamin D,serum creatinine/cystatin C ratio and ifree triiodothyronine were significantly lower in poor physical performance group than those in good physical performance group(all P<0.05).Spearman correlation analysis showed significant relationship of serum creatinine-to-cystatin C ratio with grip strength,pace,ASMI and SPPB score.Binary logistic regression analysis showed that serum creatinine/cystatin C ratio was the independent influencing factor of physical performance.Conclusions Serum creatinine-to-cystatin C ratio is strongly associated with decreased physical performance in the community elderly.It may serve as a simple serological indicator for identifying the decline in physical performance.
