摘 要:目的探讨血浆凝集素样氧化型低密度脂蛋白受体-1(LOX-1)、内皮型一氧化氮合酶(eNOS)、游离脂肪酸(FFA)在冠状动脉慢血流现象(CSFP)中的作用。方法选择2021年8月至2022年7月于咸宁市中心医院心内科进行冠状动脉造影(CAG)检查的229例患者作为研究对象,根据CAG检查结果,分为CSFP组(n=118)和对照组(CAG检查正常,n=111)。血浆LOX-1和eNOS采用双抗体夹心酶联免疫吸附法(ELISA)测定,血浆FFA水平测定采用可见分光光度法。并记录患者的心肌梗死溶栓试验帧数(TFC)。结果与对照组比较,CSFP组血浆LOX-1、FFA、平均(m)TFC均显著升高(P<0.05);此外,血浆eNOS显著降低(P<0.001)。经Spearman相关性分析,血浆LOX-1(r=0.577)、FFA(r=0.733)与mTFC呈显著正相关(P<0.001);eNOS(r=-0.342,P<0.001)与mTFC呈显著负相关。血浆LOX-1、eNOS及FFA预测CSFP的浓度为179.422 pg/ml、69.700 nmol/dl、5.335 pg/ml,特异度分别为0.694、0.500、0.892,此外三者预测CSFP的灵敏度均>0.7。Logistic多因素回归模型显示血浆LOX1、eNOS、FFA是CSFP的独立预测因子(P<0.05)。结论CSFP患者血浆LOX-1和FFA浓度升高,而血浆eNOS水平显著降低,这三种生物标志物可用于CSFP的诊断。Objective To investigate the role of plasma lectin-like oxidized low-density lipoprotein receptor-1(LOX-1),endothelial nitric oxide synthase(eNOS)and free fatty acids(FFA)in coronary slow blood flow(CSFP).Methods A total of 229 patients who underwent coronary angiography(CAG)examination in the Department of Cardiology of Xianning Central Hospital from August 2021 to July 2022 were taken as the study subjects.According to the results of CAG examination,the patients were divided into CSFP group(n=118)and control group(normal CAG examination,n=111).Plasma LOX-1 and eNOS were determined by double-antibody sandwich enzyme-linked immunosorbent assay(ELISA),and plasma FFA levels were determined by visible spectrophotometry.The number of thrombolysis test frames(TFC)of patients with myocardial infarction was recorded.Results Compared with the control group,the CSFP group had significantly increased plasma LOX1,FFA,and mean(m)TFC(P<0.05),and plasma eNOS decreased significantly(P<0.001).Spearman correlation analysis showed that plasma LOX-1(r=0.577)and FFA(r=0.733)were significantly positively correlated with mTFC(P<0.001),while eNOS(r=-0.342,P<0.001)was negatively correlated with mTFC.The concentrations of plasma LOX-1,eNOS and FFA in predicting CSFP were 179.422 pg/ml,69.700 nmol/dl and 5.335 pg/ml,with specificity of 0.694,0.500 and 0.892,respectively,and the sensitivity of the three in predicting CSFP was>0.7.Logistic multivariate regression model showed that plasma LOX-1,eNOS and FFA were independent predictors of CSFP(P<0.05).Conclusion Plasma LOX-1 and FFA concentrations were increased in CSFP patients,while plasma eNOS levels were significantly reduced,and these three biomarkers could be used for the diagnosis of CSFP.
