七氟烷联合瑞芬太尼对颅内动脉瘤手术患者呼吸功能及神经功能的影响  

Effects of Sevoflurane Combined with Remifentanil on Respiratory Function and Nerve Function in Patients Undergoing Intracranial Aneurysm Surgery

摘  要:目的:探讨七氟烷联合瑞芬太尼对颅内动脉瘤手术患者呼吸功能及神经功能的影响。方法:选取2022年3月—2024年3月中山市人民医院收治的90例颅内动脉瘤手术患者为研究对象,根据维持麻醉用药不同分为两组。对照组(n=45)采用丙泊酚联合瑞芬太尼维持麻醉,观察组采用七氟烷联合瑞芬太尼维持麻醉。对比两组血流动力学、呼吸功能、神经功能、术后不良反应发生情况。结果:T_(1)、T_(2)、T_(3)时,观察组平均动脉压(MAP)、心率(HR)均较对照组高,差异均有统计学意义(P<0.05);T_(1)、T_(2)、T_(3)时,观察组呼气末二氧化碳分压(PETCO_(2))均较对照组低,经皮动脉血氧饱和度(SpO_(2))水平均较对照组高,差异均有统计学意义(P<0.05);两组术后不良反应发生率比较,差异无统计学意义(χ^(2)=0.211,P=0.645);观察组术后3 d神经元特异性烯醇化酶(NSE)水平低于对照组,脑源性神经营养因子(BDNF)水平高于对照组,差异均有统计学意义(P<0.05)。结论:七氟烷联合瑞芬太尼用于颅内动脉瘤手术患者,对血流动力学、呼吸功能及神经功能的影响更小,且有良好的安全性。Objective:To investigate the effects of Sevoflurane combined with Remifentanil on respiratory function and nerve function in patients undergoing intracranial aneurysm surgery.Method:A total of 90 patients underwent intracranial aneurysm surgery admitted to Zhongshan City People's Hospital from March 2022 to March 2024 were selected as the study objects and divided into two groups according to different medication used for maintenance of anesthesia.The control group(n=45)was treated with Propofol combined with Remifentanil,and the observation group was treated with Sevoflurane combined with Remifentanil.The hemodynamics,respiratory function,nerve function and postoperative adverse reactions were compared between the two groups.Result:At T_(1)、T_(2) and T_(3),mean arterial pressure(MAP)and heart rate(HR)in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).At T_(1)、T_(2) and T_(3),the partial pressure of endtidal carbon dioxide(PETCO_(2))in observation group was lower than that in control group,and the percutaneous arterial oxygen saturation(SpO_(2))levels were higher than those in control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of postoperative adverse reactions between the two groups(χ^(2)=0.211,P=0.645).The neuron-specific enolase(NSE)level 3 days after surgery in the observation group was lower than that in the control group,and brain-derived neurotrophic factor(BDNF)level was higher than that in the control group,the differences were statistically significant(P<0.05).Conclusion:Sevoflurane combined with Remifentanil has less effect on hemodynamics,respiratory function and nerve function in patients undergoing intracranial aneurysm surgery,and has a good safety.
