摘　　要：目的:探究多巴酚丁胺联合硝普钠对冠心病并发慢性心力衰竭患者心功能及生活质量的影响。方法:选取2019年5月—2023年8月江南大学附属中心医院心血管内科收治的102例冠心病合并慢性心力衰竭患者作为研究对象,按随机数字表法分为参照组(n=51)和试验组(n=51)。参照组采用硝普钠治疗,试验组采用多巴酚丁胺联合硝普钠治疗,对比两组治疗前后心功能指标、加拿大心血管协会(CCS)分级、西雅图心绞痛量表(SAQ)和中国心血管病人生活质量评定问卷(CQQC)评分。结果:治疗后试验组血清脑钠肽(BNP)、左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)水平均低于参照组(P<0.05),左心室射血分数(LVEF)高于参照组(P<0.05)。治疗后试验组CCS分级优于参照组,SAQ及CQQC评分均高于参照组(P<0.05)。结论:多巴酚丁胺联合硝普钠可改善冠心病合并慢性心力衰竭患者的心功能指标,并显著提升其生活质量,其治疗效果显著。Objective:To explore the effects of Dobutamine combined with Sodium Nitroprusside on cardiac function and quality of life in patients with coronary heart disease complicated by chronic heart failure.Method:From May 2019 to August 2023,102 patients with coronary heart disease and chronic heart failure treated at the Cardiovascular Department of Jiangnan University Affiliated Central Hospital were selected as study subjects.They were divided into a reference group(n=51)and a experimental group(n=51)using a random number table method.The reference group was treated with Sodium Nitroprusside,while the experimental group was treated with Dobutamine combined with Sodium Nitroprusside.Cardiac function indexes,Canadian Cardiovascular Society(CCS)classification,Seattle angina questionaire(SAQ)and Chinese questionnaire of quality of life in patients with cardiovascular diseases(CQQC)scores before and after treatment were compared between the two groups.Result:After treatment,levels of serum brain natriuretic peptide(BNP),left ventricular end-diastolic diameter(LVEDD),and left ventricular end-systolic diameter(LVESD)in the experimental group were significantly lower than those in the reference group(P<0.05).Left ventricular ejection fraction(LVEF)was higher in the experimental group compared to the reference group(P<0.05).After treatment,the CCS classification of experimental group was better than that of reference group,and the scores of SAQ and CQQC were higher than those of reference group(P<0.05).Conclusion:Dobutamine combined with Sodium Nitroprusside can significantly improve cardiac function indicators and enhance the quality of life for patients with coronary heart disease and chronic heart failure,showing notable therapeutic effectiveness.
