摘　　要：目的:探讨桃红四物汤对Colles骨折手法复位小夹板外固定术后的影响。方法:选取2022年12月—2023年12月湖南中医药大学第二附属医院骨伤五科收治的气滞血瘀证Colles骨折患者60例为研究对象,采用随机数字表法将其分为对照组和治疗组,每组30例。对照组接受手法复位小夹板外固定治疗,治疗组在对照组基础上加用桃红四物汤治疗。比较两组患者术前、术后2周视觉模拟评分法(visual analogue scale,VAS)评分、腕关节功能(Cooney腕关节评分)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、骨形态发生蛋白-2(BMP-2)、血管内皮生长因子(VEGF)、转化生长因子-β1(TGF-β1)、中医症候评分的差异。结果:术后2周,治疗组VAS评分低于对照组,Cooney腕关节评分高于对照组(P<0.05);术后2周,治疗组炎症因子(TNF-α、IL-6)均明显低于对照组(P<0.05);术后2周,治疗组骨代谢指标(BMP-2、VEGF、TGF-β1)均高于对照组(P<0.05);术后2周,治疗组中医症候各项评分均明显低于对照组(P<0.05)。结论:对于Colles骨折患者,在手法复位小夹板外固定的基础上辨证服用桃红四物汤,疗效显著。Objective:To investigate the effect of Taohong Siwu Decoction on Colles fracture after manipulative reduction and external fixation with small splint.Method:From December 2022 to December 2023,60 patients with Colles fracture with Qi stagnation and blood stasis syndrome who were admitted to the Fifth Department of Orthopedics and Traumatology of the Second Affiliated Hospital of Hunan University of Traditional Chinese Medicine were selected as the research subjects,and they were divided into control group and treatment group by random number method,with 30 cases in each group.The control group was treated with manual reduction and external fixation of small splints,and the treatment group was treated with Taohong Siwu Decoction on the basis of the control group.The visual analogue scale(VAS)score,wrist function(Cooney wrist score),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),bone morph protein-2(BMP-2),vascular endothelial growth factor(VEGF),transforming growth factor-β1(TGF-β1),and TCM symptom score before surgery and 2 weeks after surgery were compared between the two groups.Result:2 weeks after surgery,VAS score in treatment group was lower than that in control group,Cooney wrist score was higher than that in control group(P<0.05).2 weeks after surgery,the inflammatory factors(TNF-α,IL-6)in the treatment group were significantly lower than those in the control group(P<0.05).2 weeks after surgery,bone metabolism indexes(BMP-2,VEGF,TGF-β1)in the treatment group were higher than those in the control group(P<0.05).2 weeks after surgery,the scores of TCM symptoms in the treatment group were significantly lower than those in the control group(P<0.05).Conclusion:For patients with Colles fracture,the dialectical administration of Taohong Siwu Decoction on the basis of manipulative reduction and external fixation of small splints has significant efficacy.
