摘　　要：目的:基于症状管理理论探究慢性心力衰竭(CHF)患者运动恐惧的影响因素。方法:选取2023年3月—2024年3月井冈山大学附属医院收治的200例CHF患者,根据心脏病患者运动恐惧量表(TSK-SV Heart)评估患者运动恐惧发生情况,并根据有无运动恐惧发生分为有恐惧组(>37分)、无恐惧组(≤37分)。比较两组基线资料,用logistic回归分析CHF患者运动恐惧的影响因素。结果:200例CHF患者中,72例有运动恐惧,128例无运动恐惧。有恐惧组心力衰竭症状状态问卷(SSQ-HF)评分及家庭功能中重度障碍、社会支持低水平占比均高于无恐惧组,控制态度量表(CAS-R)评分低于无恐惧组,差异均有统计学意义(P<0.05)。logistic回归分析结果显示,SSQ-HF评分、家庭功能、社会支持、CAS-R评分均为CHF患者运动恐惧的影响因素(P<0.05)。结论:CHF患者发生运动恐惧的影响因素有家庭功能、社会支持、心力衰竭症状程度及感知控制力。Objective:To explore the influencing factors of exercise fear in patients with chronic heart failure(CHF)based on symptom management theory.Method:A total of 200 patients with CHF admitted to Jinggangshan University Affiliated Hospital from March 2023 to March 2024 were selected,the occurrence of exercise fear was assessed according to the Tampa scale for kinesiophobia heart(TSK-SV Heart),and they were divided into a fear group(>37 scores)and a non-fear group(≤37 scores)based on whether there was exercise fear.The baseline data of the two groups were compared,and the influencing factors of exercise fear in CHF patients were analyzed by logistic regression.Result:Among the 200 patients with CHF,72 had exercise fear and 128 had no exercise fear.The symptom status questionnaire-heart failure(SSQ-HF)score,the proportions of moderate to severe disorders of family function and low level of social support in the fear group were higher than those in the non-fear group,and the control attitudes scale-revised(CAS-R)score was lower than that in the non-fear group,the differences were statistically significant(P<0.05).logistic regression analysis showed that SSQ-HF score,family function,social support and CAS-R score were all influencing factors of exercise fear in patients with CHF(P<0.05).Conclusion:The influencing factors of exercise fear in CHF patients are family function,social support,heart failure symptoms degree and perceived control.
