腹腔镜高位骶韧带联合圆韧带悬吊术治疗POP-Q分期Ⅱ~Ⅲ期子宫脱垂的疗效及对生活质量的影响

摘　　要：目的分析子宫脱垂的特点,总结腹腔镜高位骶韧带联合圆韧带悬吊术治疗盆腔脏器脱垂定量分期法(POP-Q)分期Ⅱ~Ⅲ期子宫脱垂患者的疗效,并评价其对患者生活质量等的影响。方法选择60例POP-Q分期Ⅱ~Ⅲ期子宫脱垂患者作为研究对象,依据随机数字表法分为对照组与观察组,每组30例。两组均行腹腔镜子宫全切术,在此基础上,对照组行高位骶韧带悬吊术治疗,观察组行高位骶韧带联合圆韧带悬吊术治疗。比较两组手术情况、生活质量[盆底障碍影响简易问卷-7(PFIQ-7)评分]、性生活质量[盆底脏器脱垂/尿失禁性功能问卷(PISQ-12)评分]、随访复发率及满意度。结果两组手术时间、出血量、住院时间以及阴道前、后壁修补率比较无差异(P>0.05)。两组术后1年PFIQ-7评分均低于术前,PISQ-12评分均高于术前,且观察组PFIQ-7评分(96.80±15.50)分低于对照组的(105.50±11.20)分,PISQ-12评分(39.60±3.10)分高于对照组的(36.80±2.20)分(P<0.05)。观察组随访复发率3.33%低于对照组的20.00%,满意度93.33%高于对照组的70.00%(P<0.05)。结论POP-Q分期Ⅱ~Ⅲ期子宫脱垂直接影响患者的性生活与生活质量,需要尽早进行手术治疗,其中,腹腔镜高位骶韧带联合圆韧带悬吊术的效果理想,能促进患者症状体征改善,减少患者复发风险,整体满意度高。Objective To analyze the characteristics of uterine prolapse,summarize the curative effect of laparoscopic high uterosacral ligament combined with round ligament suspension for patients with uterine prolapse of pelvic organ prolapse quantitation(POP-Q)stagingⅡ-Ⅲ,and evaluate its influence on the quality of life of patients.Methods 60 patients with uterine prolapse of POP-Q stagingⅡ-Ⅲwere selected as the study subjects,and were divided into control group and observation group according to random numerical table,with 30 cases in each group.Both groups underwent total laparoscopic hysterectomy.On this basis,the control group underwent round ligament suspension,and the observation group underwent high uterosacral ligament combined with round ligament suspension.Comparison of surgical status,quality of life[pelvic floor impact questionnaire short form-7(PFIQ-7)score],quality of sexual life[pelvic organ prolapse/urinary incontinence sexual questionnaire-12(PISQ-12)score],recurrence rate and satisfaction between the two groups.Results There were no differences in operation time,blood loss,length of hospital stay and repair rate of anterior and posterior vaginal walls between the two groups(P>0.05).1 year after surgery,PFIQ-7 score in both groups was lower than that before surgery,and PISQ-12 score was higher than that before surgery;the observation group had a lower PFIQ-7 score of(96.80±15.50)points than(105.50±11.20)points in the control group,and a higher PISQ-12 score of(39.60±3.10)points than(36.80±2.20)points in the control group(P<0.05).The observation group had a lower recurrence rate of 3.33%than 20.00%in the control group,and a higher satisfaction rate of 93.33%than 70.00%in the control group(P<0.05).Conclusion Uterine prolapse of POP-Q stagingⅡ-Ⅲdirectly affects the sexual life and quality of life of patients,and requires early surgical treatment.Among them,laparoscopic high uterosacral ligament combined with the round ligament suspension is ideal,which can promote the improvement of symptoms
