静脉剥脱导管手术治疗原发性大隐静脉曲张的效果及临床优势评价

摘　　要：目的分析静脉剥脱导管手术在原发性大隐静脉曲张治疗中的效果和优势。方法对63例原发性大隐静脉曲张患者的资料进行回顾性研究,根据治疗方法的不同分为观察组和对照组。观察组33例患者行静脉剥脱导管手术治疗,对照组30例患者行高位结扎联合剥脱术治疗。观察两组治疗效果、大隐静脉闭合率;对比两组术后静脉曲张残留情况(术后残留静脉曲张血管数量、直径、长度),手术前后患肢血流速度(髂外静脉、股总静脉、腘静脉),术后3 d的疼痛评分、疼痛持续时间和术后恢复时间,术后并发症发生情况及复发情况。结果两组治疗有效率和大隐静脉闭合率均为100.00%。两组术后患肢髂外静脉、股总静脉、腘静脉的血流速度较术前均有改善(P<0.05);但两组术后组间患肢血流速度及术后静脉曲张残留情况对比,差异无统计学意义(P>0.05)。观察组术后3 d的疼痛评分(2.34±0.78)分低于对照组的(2.91±0.82)分,疼痛持续时间(1.37±0.44)d、术后恢复时间(3.52±0.78)d均短于对照组的(1.98±0.52)、(4.87±0.90)d(P<0.05)。观察组术后皮下瘀血发生率3.03%低于对照组的20.00%(P<0.05)。随访12个月,观察组复发率3.03%(1/33)低于对照组的23.33%(7/30)(P<0.05)。结论在原发性大隐静脉曲张的临床治疗中,静脉剥脱导管手术与大隐静脉高位结扎联合剥脱术效果相当,同时前者能够更好地预防皮下淤血及远期复发,且该术式操作难度较小,适宜在基层医院推广。Objective To analyze the effect and advantages of vein stripping catheter surgery for primary great saphenous varicose veins.Methods The data of 63 patients with primary great saphenous varicose veins were retrospectively studied,and the cases were divided into an observation group and a control group according to different treatment methods.33 patients in the observation group were treated with vein stripping catheter surgery,and 30 patients in the control group were treated with high ligation combined with stripping.Observation of treatment effect and closure rate of saphenous varicose veins between the two groups;comparison of postoperative residual varicose veins(number,diameter and length of postoperative residual varicose veins),blood flow velocity of the affected limb(external iliac vein,common femoral vein and popliteal vein)before and after surgery,pain score at 3 d after surgery,pain duration and postoperative recovery time,postoperative complications and recurrence between the two groups.Results The effective rate and closure rate of saphenous varicose veins were 100.00%in both groups.The postoperative blood flow velocity of the external iliac vein,common femoral vein and popliteal vein of the affected limb was improved in both groups compared with before surgery(P<0.05),but there was no statistically significant difference in postoperative blood flow velocity of the affected limb and postoperative residual varicose veins between the two groups(P>0.05).The observation group had a lower pain score of(2.34±0.78)points at 3 d after surgery than(2.91±0.82)points in the control group;the observation group had pain duration of(1.37±0.44)d and postoperative recovery time of(3.52±0.78)d,which were shorter than(1.98±0.52)and(4.87±0.90)d in the control group(P<0.05).The incidence of subcutaneous stasis in the observation group was 3.03%,which was lower than 20.00%in the control group(P<0.05).After 12 months of follow-up,the recurrence rate of the observation group was 3.03%(1/33),lower than 23.33%(7/30)of
