苯磺酸左旋氨氯地平联合氯沙坦钾氢氯噻嗪治疗老年高血压的疗效及对PⅢNP、NT-proBNP水平的影响

摘　　要：目的探讨苯磺酸左旋氨氯地平联合氯沙坦钾氢氯噻嗪治疗老年高血压的有效性、安全性,以及对Ⅲ型前胶原氨基端原肽(PⅢNP)、N端脑钠肽前体(NT-proBNP)水平的影响。方法将80例老年高血压患者纳入此次研究,采用随机数字表法将患者分为血管紧张素受体拮抗剂(ARB)+氢氯噻嗪(HCTZ)治疗组和钙离子通道阻滞剂(CCB)+ARB+HCTZ治疗组,每组40例。ARB+HCTZ治疗组患者给予氯沙坦钾氢氯噻嗪片治疗,CCB+ARB+HCTZ治疗组在ARB+HCTZ治疗组的基础上给予苯磺酸左旋氨氯地平治疗。比较两组临床疗效,治疗前后NT-proBNP、PⅢNP水平,不良反应发生率。结果CCB+ARB+HCTZ治疗组临床治疗总有效率(100.00%)高于ARB+HCTZ治疗组(85.00%)(P<0.05)。治疗后,两组NT-proBNP和PⅢNP水平均较治疗前显著降低,且CCB+ARB+HCTZ治疗组NT-proBNP(250.63±24.54)pg/ml和PⅢNP(41.44±5.29)μg/L明显低于ARB+HCTZ治疗组的(312.37±27.91)pg/ml、(46.52±5.31)μg/L(P<0.05)。两组不良反应发生率比较差异不具有统计学意义(P>0.05)。结论苯磺酸左旋氨氯地平联合氯沙坦钾氢氯噻嗪治疗老年高血压安全有效,且可有效逆转降低PⅢNP、NT-proBNP水平,改善心功能。Objective To explore the effectiveness and safety of levamlodipine besylate combined with losartan potassium and hydrochlorothiazide in the treatment of senile hypertension and its influence on the levels of amino-terminal propeptide of typeⅢprocollagen(PⅢNP)and N-terminal pro-brain natriuretic peptide(NT-proBNP).Methods 80 senile patients with hypertension were included in this study,and the patients were divided into angiotensinⅡ-receptors antagonist(ARB)+hydrochlorotiazide(HCTZ)group and calcium channel blocker(CCB)+ARB+HCTZ group by random numerical table,with 40 cases in each group.Patients in the ARB+HCTZ treatment group were treated with losartan potassium and hydrochlorothiazide tablets,and patients in the CCB+ARB+HCTZ treatment group were treated with levamlodipine besylate based on the ARB+HCTZ group.The clinical efficacy,NT-proBNP and PⅢNP levels before and after treatment,and the incidence of adverse reactions were compared between the two groups.Results The total clinical effective rate of CCB+ARB+HCTZ group(100.00%)was higher than that of ARB+HCTZ group(85.00%)(P<0.05).After treatment,the levels of NT-proBNP and PⅢNP in both groups were significantly lower than those before treatment;CCB+ARB+HCTZ group had NT-proBNP of(250.63±24.54)pg/ml and PⅢNP of(41.44±5.29)μg/L,which were significantly lower than(312.37±27.91)pg/ml and(46.52±5.31)μg/L in ARB+HCTZ group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Levamlodipine besylate combined with losartan potassium and hydrochlorothiazide is safe and effective in the treatment of senile hypertension,and can effectively reverse the levels of PⅢNP and NT-proBNP,and improve cardiac function.
