四君子合温胆汤加减方治疗痛风性肾病的临床疗效分析

摘　　要：目的研讨四君子合温胆汤加减方治疗痛风性肾病的临床疗效。方法96例痛风性肾病患者,以电脑随机法分为优质组和参照组,每组48例。优质组采用四君子合温胆汤加减方治疗,参照组采用常规治疗+非布司他片治疗。比较两组患者治疗前后总胆固醇(TC)、甘油三脂(TG)、低密度脂蛋白胆固醇(LDL-C)、血尿酸(SUA)、血肌酐(Scr)、尿素氮(BUN)、胱抑素C(Cys-C)、主要症状评分及治疗效果。结果治疗后,优质组患者TC(4.39±0.85)mmol/L、TG(1.66±1.09)mmol/L、LDL-C(2.72±0.76)mmol/L低于参照组的(5.14±0.87)、(2.45±0.84)、(3.67±0.78)mmol/L,差异有意义(P<0.05)。优质组患者治疗总有效率为97.92%(47/48),高于参照组的72.92%(35/48),差异有意义(P<0.05)。治疗后,优质组SUA(393.26±58.49)μmol/L、Cys-C(1.18±0.15)mg/L、BUN(3.99±0.93)mmol/L、Scr(91.15±16.42)μmol/L均低于参照组的(435.95±69.27)μmol/L、(1.27±0.18)mg/L、(4.99±1.26)mmol/L、(100.35±17.78)μmol/L,差异有意义(P<0.05)。治疗后,优质组患者关节红肿评分(1.22±0.37)分、关节疼痛评分(1.19±0.36)分、下肢水肿评分(1.19±0.53)分均低于参照组的(2.16±0.29)、(2.22±0.31)、(1.99±0.34)分,差异有意义(P<0.05)。结论采用四君子合温胆汤加减方治疗痛风性肾病患者能够取得较高疗效,改善患者血脂、肾功能及临床症状,值得推广。Objective To study the clinical efficacy of modified Sijunzi and Wendan decoction in the treatment of gouty nephropathy.Methods 96 patients with gouty nephropathy were divided into quality group and reference group by computer randomization method,with 48 cases in each group.The quality group was treated with modified Sijunzi and Wendan decoction,and the reference group was treated with conventional treatment plus febuxostat tablets.Both groups were compared in terms of total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),serum uric acid(SUA),serum creatinine(Scr),blood urea nitrogen(BUN),cystatin C(Cys-C)and main symptom score before and after treatment,and therapeutic effect.Results After treatment,the quality group had TC of(4.39±0.85)mmol/L,TG of(1.66±1.09)mmol/L and LDL-C of(2.72±0.76)mmol/L,which were lower than(5.14±0.87),(2.45±0.84)and(3.67±0.78)mmol/L in the reference group.The difference was significant(P<0.05).The total effective rate of the quality group was 97.92%(47/48),which was higher than 72.92%(35/48)of the reference group,and the difference was significant(P<0.05).After treatment,the quality group had SUA of(393.26±58.49)μmol/L,Cys-C of(1.18±0.15)mg/L,BUN of(3.99±0.93)mmol/L,and Scr of(91.15±16.42)μmol/L,which were lower than(435.95±69.27)μmol/L,(1.27±0.18)mg/L,(4.99±1.26)mmol/L,and(100.35±17.78)μmol/L in the reference group,and the difference was significant(P<0.05).After treatment,the joint swelling score,joint pain score and lower limb edema score in the quality group were(1.22±0.37),(1.19±0.36)and(1.19±0.53)points,which were lower than(2.16±0.29),(2.22±0.31)and(1.99±0.34)points in the reference group.The difference was significant(P<0.05).Conclusion Modified Sijunzi and Wendan decoction has achieved high efficacy in the treatment of gouty nephropathy patients,and can improve blood lipids,renal function and clinical symptoms,and is worthy of promotion.
