椎板间隙入路脊柱内镜技术治疗老年腰椎间盘突出症的临床观察

摘　　要：目的分析椎板间隙入路脊柱内镜技术对老年腰椎间盘突出症患者的治疗效果。方法从2023年5月—2024年5月本院接收的腰椎间盘突出症病例中,随机抽取80例患者进行研究,通过数字表随机排序的方法,将其划分为对照组(40例,采用牵引治疗技术)与观察组(40例,采用椎板间隙入路脊柱内镜技术)。分析两组患者治疗前后的疼痛缓解情况、腰椎功能状态变化、临床症状的改进程度、心理状态以及生活质量。结果观察组治疗效果优于对照组(P<0.05);观察组视觉模拟评分法(VAS)评分低于对照组(P<0.05);观察组Oswestry功能障碍指数(ODI)评分较对照组更低(P<0.05);对治疗后临床症状改善情况进行分析对比得知,观察组症状改善情况较对照组更佳(P<0.05)。观察组焦虑自评量表(SAS)评分、抑郁自评量表(SDS)评分均低于对照组,生活质量量表(SF-36)评分高于对照组(P<0.05)。结论椎板间隙入路脊柱内镜技术对老年腰椎间盘突出症的治疗效果突出,能有效缓解疼痛,增强腰椎活动性,提升症状改善,加速康复进程。Objective To analyze the therapeutic effect of spinal endoscopic technique through the intervertebral disc space approach on elderly patients with lumbar disc herniation.Methods From May 2023 to May 2024,80 patients with lumbar disc herniation received by our hospital were randomly selected for study,and the numerical table was randomly divided into control group(40 cases,traction treatment)and observation group(40 cases,spinal endoscopic technique of laminar space approach).The pain relief,changes in lumbar spine functional status,improvement of clinical symptoms,psychological state and quality of life were analyzed before and after treatment in the two groups.Results The treatment effect of the observation group was better than that of the control group(P<0.05),the score of visual analogue scoring(VAS)in the observation group was lower than that of the control group(P<0.05),and the score of Oswestry dysfunction index(ODI)in<the observation group was lower than that of the control group(P<0.05).The scores of anxiety self-rating scale(SAS)and depression self-rating scale(SDS)in the observation group were lower than those in the control group,and the scores of quality of life scale(SF-36)were higher than thos e in the control group(P<0.05).Conclusion The spinal endoscopic technique through the intervertebral disc space approach has outstanding therapeutic effects on elderly patients with lumbar disc herniation,effectively relieving pain,enhancing lumbar mobility,improving symptoms,and accele rating rehabilitation process.
