摘 要:目的:探究影响重症监护病房(ICU)护理人员道德困扰的相关因素,并对规范化培训进行分析。方法:选取张掖市二级、三级医院ICU护理人员共计125名,选取时间:2023年1月—2024年6月。通过自制一般资料问卷、护士道德困境量表(MDS)进行调查,收集基础信息,分析造成ICU护理人员道德困扰的影响因素,并进一步讨论规范化培训措施的作用。结果:不同年龄、ICU工作年限、职称、聘用形式ICU护理人员与道德困境量表(MDS)评分对比(P<0.05),其中ICU工作年限≥10年MDS评分(43.68±31.49)分,年龄41~50岁评分(41.59±34.08)分,主管护师及以上职称(49.74±40.22)分,合同制聘用形式评分(43.13±28.94)分,在其他同类评价项目中评分最高(P<0.05);多元线性回归分析结果显示,ICU工作年限、聘用形式、职称为ICU护理人员道德困扰的独立影响因素(P<0.05)。结论:ICU护理人员受道德困扰较深,医院应通过改善工作环境、加强伦理道德知识规范培训,以降低护理人员道德困境。Objective:To explore the related factors affecting the moral distress of intensive care unit(ICU)nursing staff,and analyze the standardized training.Methods:A total of 125 ICU nurses were selected from secondary and tertiary hospitals in Zhangye city from January 2023 to June 2024.Through self-made general data questionnaire and nurses’moral distress scale(MDS),the basic information was collected,the influencing factors of ICU nurses’moral distress were analyzed,and the effect of standardized training measures was further discussed.Results:Nursing staff in ICU with different age,years of work in ICU,job title,employment form and moral dilemma scale(MDS)score were compared(P<0.05),among which the MDS score was 43.68±31.49 for those who work for 10 years or more in ICU and 41.59±34.08 for ages 41 to 50.Those with supervisor and above titles have 49.74±40.22 points,those employed in contract form have 43.13±28.94 points,the highest score in other similar evaluation items(P<0.05);The results of multiple linear regression analysis showed that the working years,employment forms and professional titles of ICU nurses were independent influencing factors of moral distress(P<0.05).Conclusion:Nursing staff in ICU are deeply troubled by morality.Hospitals should improve the working environment and strengthen the training of ethical knowledge to reduce the moral dilemma of nursing staff.
