标准化产前超声筛查在胎儿心脏畸形诊断中的应用

摘　　要：目的:探析标准化产前超声筛查在胎儿心脏畸形诊断中的应用价值。方法:选择2023年1月—2024年6月红河州滇南中心医院收治的疑似胎儿心脏畸形的产妇80例,所有患者均接受常规产前超声三步筛查法,以及五步标准化产前超声筛查法,并以分娩后心脏彩超结果为金标准,对比不同产前超声筛查所具临床诊断价值。结果:金标准结果中阳性有66例;常规产前超声三步筛查法结果中阳性有44例,而五步标准化产前超声筛查法结果中阳性有63例;五步标准化产前超声筛查法灵敏度94.45%(63/66),特异性85.71%(12/14),以及准确率93.75%(75/80),相比常规产前超声三步筛查法灵敏度66.67%(44/66),特异性57.14%(8/14),以及准确率65.00%(52/80),明显较高(P<0.05);五步标准化产前超声筛查法在胎儿心脏畸形不同类型中检出率,相比常规产前超声三步筛查法明显较高(P<0.05)。结论:目前临床胎儿心脏畸形诊断中,五步标准化产前超声筛查法所具诊断价值较高,可对不同类型胎儿心脏畸形进行诊断,为后续干预提供支持。Objective:To explore the application value of standardized prenatal ultrasound screening in the diagnosis of fetal heart malformation.Methods:A total of 80 pregnant women with suspected fetal heart malformation admitted to Yunnan Honghe Prefecture Central Hospital from January 2023 to June 2024 were selected.All patients received routine t hree-step p renatal u ltrasound s creening m ethod a nd f ive-step s tandardized p renatal u ltrasound s creening method.The clinical diagnostic value of different prenatal ultrasound screening methods was compared with the gold standard of cardiac color ultrasound results after delivery.Results:In the gold standard results,66 cases were positive.44 cases were positive in the results of routine three-step prenatal ultrasound screening,while 63 cases were positive in the results of five-step standardized prenatal ultrasound screening.The sensitivity,specificity and accuracy of five-step standardized prenatal ultrasound screening method was 94.45%(63/66),85.71%(12/14),and 93.75%(75/80)respectively,which were significantly higher than 66.67%(44/66),57.14%(8/14),and 65.00%(52/80)of conventional three-step prenatal ultrasound screening method(P<0.05).The detection rate of five-step standardized prenatal ultrasound screening in different types of fetal heart malformations was significantly higher than that of conventional three-step prenatal ultrasound screening(P<0.05).Conclusion:The five-step standardized prenatal ultrasound screening method has high diagnostic value in the diagnosis of fetal heart malformations,which can diagnose different types of fetal heart malformations and provide support for follow-up intervention.
