胸部X线片在大学生肺结核诊断中的效能及其临床价值探讨

摘　　要：目的 探讨胸部X线片在大学生肺结核诊断中的效能及其临床价值。方法 回顾性分析了2019年3月—2024年3月在本院接受胸部X线检查及治疗的77例肺结核患者,在这些患者的痰液或病灶中检出了结核分枝杆菌。所有患者均进行了胸部X线正位和侧位摄影。比较正位胸部X线片与侧位胸部X线片对肺结核的诊断灵敏度、特异度、准确率。结果 以痰液或病灶中发现结核分枝杆菌(金标准)为参考,确诊为肺结核患者共77例;其中正位胸部X线片诊出71例,占92.21%,侧位胸部X线片诊出73例,占94.81%;正位胸部X线片诊断肺结核的灵敏度、特异度及准确率分别为99.42%、33.33%、96.64%,侧位胸部X线片诊断肺结核的灵敏度、特异度及准确率分别为99.71%、46.15%、97.76%,两者灵敏度、特异度及准确率比较差异均无统计学意义(P>0.05)。结论 正位胸部X线片与侧位胸部X线片对肺结核的诊断灵敏度、准确率均较为理想,能有效应用于大学生肺结核诊断及病情监测。Objective To investigate the efficacy and clinical value of chest X-ray in the diagnosis of pulmonary tuberculosis in college students.Methods The clinical data of 77 patients with tuberculosis who were treated in our hospital from March 2019 to March 2024 with mycobacterium tuberculosis found in sputum or lesions were retrospectively analyzed.All patients were treated with anterior and lateral chest X-rays.The sensitivity,specificity and accuracy of the diagnosis of pulmonary tuberculosis were compared between anteroposternal chest X-ray and lateral chest X-ray.Results A total of 77 patients were diagnosed as pulmonary tuberculosis based on Mycobacterium tuberculosis found in sputum or lesions (gold standard).Among them,71 cases (92.21%) were diagnosed with anteroposterior chest X-ray,and 73 cases (94.81%) were diagnosed with lateral chest X-ray.The sensitivity,specificity and accuracy were 99.71%,46.15% and 97.76%,respectively,and there was no statistical significance in the sensitivity,specificity and accuracy of anterograde chest X-ray and lateral chest X-ray in the diagnosis of pulmonary tuberculosis (P>0.05).Conclusion The sensitivity and accuracy of orthographic chest X-ray and lateral chest X-ray in the diagnosis of pulmonary tuberculosis are ideal,and can be effectively used in the diagnosis and monitoring of pulmonary tuberculosis in college students.
