腹腔镜胆囊切除术患者实施依托咪酯持续输注对细胞因子和肾上腺皮质功能的影响

摘  要:目的 分析腹腔镜胆囊切除术患者持续输注依托咪酯对细胞因子、肾上腺皮质功能的影响。方法 选取2023年7月—2024年7月在本院采取腹腔镜胆囊切除术治疗的80例患者为研究对象,并随机将其分为对照组和观察组,每组40例。其中,对照组采用七氟烷吸入、观察组采用依托咪酯持续输注进行维持麻醉,术后进行3 d的随访,对比两组干预效果。结果 在T1、T2时刻,观察组的皮质醇、醛固醇、促肾上腺皮质激素波动幅度明显比对照组大(P<0.05);T3时刻两组患者的各项肾上腺皮质功能对比差异较小,并且其水平与T0时刻较为相近(P>0.05)。结论 对接受腹腔镜胆囊切除术的患者而言,持续输注依托咪酯具有显著的疗效。然而,该药物在短期内会对患者的肾上腺皮质功能产生较大刺激,导致功能出现显著波动。值得注意的是,24 h后患者的皮质功能将有所恢复。Objective To analyze the effect of continuous infusion of etomidate on cytokine and adrenal cortex function in patients undergoing laparoscopic cholecystectomy.Methods The subjects of the study were 80 patients treated with laparoscopic cholecystectomy in our hospital,the case enrollment time was July 2023~July 2024,and they were randomly divided into two groups,the control group and the observation group were treated with sevoflurane inhalation and etomidate continuous infusion for maintenance anesthesia,and 3D follow-up was performed after surgery,and the intervention effect of the two groups was compared.Results The fluctuations of cortisol,aldosterol and adrenocorticotropic hormone in the observation group were significantly larger than those in the control group at T1 and T2 (P<0.05),and the adrenal cortex function between the two groups was slightly different from that at T3,and their levels were similar to those at T0 (P>0.05).Conclusion The effect of continuous infusion of etomidate in patients undergoing laparoscopic cholecystectomy is significant,but it will cause greater stimulation of the adrenal function of the patients in a short period of time,resulting in greater fluctuations,and the cortical function will recover after 24 hours.
