摘　　要：目的 分析多囊卵巢综合征(PCOS)不孕症患者血清中白细胞介素17A(IL-17A)、补体C1q/肿瘤坏死因子相关蛋白-3(CTRP3)的表达情况,及二者与体外受精-胚胎移植(IVF-ET)妊娠情况的相关性。方法 将2021年2月至2024年2月大同市第一人民医院收治的159例PCOS不孕症患者作为研究对象。按照患者接受IVF-ET治疗后的妊娠情况,将其分为妊娠成功组(n=84)和妊娠失败组(n=75)。比较两组患者临床资料,检测各组患者血清中IL-17A、CTRP3的水平。Pearson相关分析探讨PCOS不孕症患者血清IL-17A、CTRP3水平与性激素水平的相关性;Logistic回归分析影响患者IVF-ET妊娠结局的关键因素;受试者工作特征(ROC)曲线评估血清IL-17A、CTRP3水平对患者IVF-ET妊娠结局的预测价值。结果 与妊娠成功组比较,妊娠失败组的血清IL-17A、黄体生成素(LH)、睾酮(T)水平升高(P<0.05),而血清CTRP3、卵泡刺激素(FSH)水平降低(P<0.05)。Pearson结果显示,PCOS不孕症患者血清IL-17A水平与LH、T水平呈正相关(r=0.534、0.648,P<0.05),与FSH水平呈负相关(r=-0.664,P<0.05);血清CTRP3水平则与LH、T水平呈负相关(r=-0.558、-0.522,P<0.05),与FSH水平呈正相关(r=0.644,P<0.05)。Logistic分析显示,血清IL-17A、LH、T水平是影响患者IVF-ET妊娠失败的危险因素,血清CTRP3、FSH水平则为保护因素(P<0.05);血清IL-17A、CTRP3单独及联合检测的曲线下面积(AUC)为0.810(95%CI:0.720~0.881)、0.850(95%CI:0.765~0.913)、0.918(95%CI:0.847~0.963),联合检测优于二者单独检测(Z二者联合-IL-17A=3.112、Z二者联合-CTRP3=2.514,P<0.05)。结论 PCOS不孕症患者血清IL-17A高表达,CTRP3低表达,二者与患者IVF-ET妊娠结局显著相关。Objective To analyze the serum levels of interleukin-17A(IL-17A)and complement C1q/tumor necrosis factor related protein-3(CTRP3)in patients with polycystic ovary syndrome(PCOS)infertility,and their correlation with the pregnancy outcome of in vitro fertilization-embryo transfer(IVF-ET).Methods A total of 159 patients with PCOS infertility admitted to the First People's Hospital of Datong from February 2021 to February 2024 were included as the study subjects.They were divided into a successful pregnancy group(n=84)and a failed pregnancy group(n=75)according to their pregnancy outcomes of IVF-ET.The clinical data of both groups were collected and compared.The serum levels of IL-17A and CTRP3 were detected.Pearson correlation analysis was utilized to explore the correlation between serum IL-17A and CTRP3 levels and sex hormone levels in patients with PCOS infertility.Logistic regression was applied to analyze the key factors affecting the pregnancy outcomes of patients undergoing IVF-ET.Receiver operating characteristic(ROC)curves were plotted to evaluate the predictive value of serum IL-17A and CTRP3 levels for IVF-ET pregnancy outcomes in PCOS infertility patients.Results Compared with the successful pregnancy group,the failed pregnancy group exhibited increased serum levels of IL-17A,luteinizing hormone(LH),and testosterone(T),but decreased serum levels of CTRP3 and follicle-stimulating hormone(FSH)(P<0.05).Pearson correlation analysis revealed that in PCOS infertility patients,serum IL-17A levels were positively correlated with LH and T levels(r=0.534,0.648,P<0.05),but negatively correlated with FSH levels(r=-0.664,P<0.05),while serum CTRP3 levels were negatively correlated with LH and T levels(r=-0.558,-0.522,P<0.05),but positively correlated with FSH levels(r=0.644,P<0.05).Logistic regression analysis showed that serum IL-17A,LH,and T levels were risk factors for IVF-ET pregnancy failure in patients,while serum CTRP3 and FSH levels were protective factors(P<0.05).The areas under the curve(AUCs)of serum IL-17
