摘  要:目的 探讨3D打印个性化截骨模块(PSI)导板和模拟手术辅助膝关节单髁置换术(UKA)的临床治疗效果。方法 收集2021年1月至2023年12月连云港市第二人民医院骨科收治的41例重度膝关节骨性关节炎患者。采用随机数字表法将OA患者随机分为对照组(传统UKA,n=20)和PSI组(3D打印个性化截骨导板,n=21)。记录术后不同假体的各个角度与目标值的偏差值。采用膝关节活动度和膝关节功能评分(HSS)对手术的临床治疗效果进行术后评价。结果 对照组的B角、股骨假体及假体总体的偏离评分显著高于PSI组(P<0.05)。膝关节活动度、HSS评分在术前、术后3个月、6个月和12个月差异有统计学意义(P<0.001)。与术前相比,对照组和PSI组末次随访的膝关节活动度、HSS评分均显著改善,差异有统计学意义(P<0.05)。A角偏离评分与末次随访时HSS评分存在负相关(r=-0.379,P=0.015)。结论 采用3D打印PSI导板技术和模拟手术辅助UKA可以有效提高胫骨假体和股骨假体的精准性,且创伤小、患者恢复快,临床应用效果良好。Objective To investigate the clinical treatment effect of 3D printed personalized osteotomy module(PSI)guide plate and simulated surgery assisted unicompartmental knee arthroplasty(UKA).Methods 41 patients with severe knee osteoarthritis(OA)admitted to the Orthopedics Department of Lianyungang Second People's Hospital from January 2021 to December 2023 were collected.Using a random number table method,OA patients were randomly divided into a control group(traditional UKA,n=20)and a PSI group(3D printed personalized osteotomy guide plate,n=21).The deviation values between the various angles of different prostheses and the target values after surgery were recorded.The clinical treatment effect of surgery using knee joint range of motion and knee joint function score(HSS)after surgery were evaluated.Results The B angle,femoral prosthesis,and overall deviation score of the prosthesis in the control group were significantly higher than those in the PSI group(P<0.05).There were statistically significant differences(P<0.001)in knee joint range of motion and HSS score before surgery,3 months,6 months,and 12 months after surgery.Compared with preoperative results,the knee joint range of motion and HSS score in the control group and PSI group showed significant improvement at the last follow-up(P<0.05).There is a significant negative correlation between the A angle deviation score and the HSS score at the last follow-up(r=-0.379,P=0.015).Conclusion The application of 3D printed PSI guide plate technology and simulated surgery assisted UKA can effectively improve the accuracy of tibial and femoral prostheses,with minimal trauma,fast patient recovery,and good clinical application effects.
