摘 要:脊髓损伤(SCI)导致神经通路中断,引发不可逆的神经功能障碍,神经修复与再生一直是SCI患者康复的关键目标。然而如何在临床康复中实现更有效和精准神经修复,是目前该领域亟待解决的问题之一。无创神经调控技术作为一种操作简便且副作用少的技术,能够诱发脊柱上结构及脊髓神经回路有益的适应性变化,为SCI患者恢复失去功能、减少并发症和提高生活质量带来了希望。文章就无创神经调控技术治疗SCI的研究证据进行综述,并依据技术类别,从参数设置、作用机制及临床应用前景三个方面系统梳理了其研究进展。未来可应用人工智能技术进行刺激参数的优化,并实现多靶点、多模式的治疗策略,使用大规模的临床试验进行验证,以造福更多的SCI患者。Spinal cord injury(SCI)leads to the interruption of neural pathways and results in irreversible neural dysfunction.Neural repair and regeneration are critical goals of rehabilitation for SCI.However,achieving effective and precise neural repair in clinical rehabilitation remains a pressing challenge in this field.Non-invasive neuromodulation techniques,as a simple and easy-to-operate technology with few side effects,can induce beneficial adaptive changes in supraspinal structures and spinal cord neural circuits,bringing hope for SCI patients to recover lost functions,reduce complications and improve their quality of life.This article reviews the research progress on non-invasive neuromodulation techniques for SCI treatment,and systematically sorts out its research progress from three aspects:parameter setting,mechanism of action,and clinical application prospects,based on the technology category.In the future,to benefit more SCI patients,artificial intelligence technology can be used to optimize stimulation parameters and implement multi-target,multi-modal treatment strategies,which can be verified using large-scale clinical trials.
