高脂血症对肩袖损伤的影响机制及研究进展

摘　　要：随着饮食结构和生活方式的变化,高脂血症发病率逐年增高,正在成为当代中国社会的一种常见疾病。肩袖损伤是一种以肌腱损伤为主要表现的肩关节常见疾病。高脂血症患者肩袖损伤发病率高,预后较差,因而展开对上述两种疾病之间的关联性研究十分重要。高脂血症主要通过脂蛋白水平对肩袖损伤产生影响。高脂血症促进LDL转入细胞外基质,通过核因子κB途径启动炎症反应发生,通过JAK2/STAT3及NOD样受体热蛋白结构域相关蛋白3通路维持炎症状态,介导肩袖内持续的低度炎症反应与细胞外基质降解,最终破坏肩袖的稳定性并影响肩袖损伤及修复。肩袖修复术后,高脂血症对于炎症反应、脂肪浸润、肌肉萎缩的促进作用导致肩袖损伤创面愈合不良。文章从高脂血症对肩袖损伤发病率的影响与机制及术后预后影响两方面进行综述。With the change of people's eating habits and lifestyles,the incidence of hyperlipidemia is continuously increasing,and is becoming a common disease in contemporary Chinese society.Rotator cuff injury is a common shoulder disease of the shoulder joint,mainly manifested by tendon injury.Hyperlipidemia is an important factor affecting the onset and prognosis of rotator cuff injury,so it is important to conduct research on the correlation between them.Hyperlipidemia mainly affects rotator cuff joint injury through lipoprotein levels.Hyperlipidemia promotes the transfer of LDL into the extracellular matrix,initiates inflammation through the NF-kB pathway,maintains inflammation through the JAK2/STAT3 and NLRP3 pathways,mediates sustained low-grade inflammation and extracellular matrix degradation within the rotator cuff joint,ultimately disrupting the stability of the rotator cuff joint and affecting rotator cuff injury and repair.After rotator cuff repair surgery,hyperlipidemia promotes inflammation,fat infiltration,and muscle atrophy,leading to poor wound healing of rotator cuff injuries.This article reviews the influence and mechanism of hyperlipidemia on the incidence rate of rotator cuff injury and the influence of postoperative prognosis.
