全人照护管理模式对PICU住院重症肺炎患儿治疗依从性及肺功能的影响

摘　　要：探究全人照护管理模式对儿科重症监护病房(PICU)住院重症肺炎患儿治疗依从性及肺功能的影响。方法 选择2023年1月至12月于苏州大学附属儿童医院PICU收治的重症肺炎患儿90例作为研究对象,按随机数字表法将其分为研究组(45例)和对照组(45例),对照组患儿予以常规护理干预,而研究组患儿采用全人照护管理模式。对比两组重症肺炎患儿的临床症状消退所需时间与住院时间、治疗依从性及肺功能情况。结果 研究组患儿的住院时间及临床症状消退所需时间短于对照组(P<0.05);干预后,两组患儿SAI-治疗依从性评分均高于同组干预前,且研究者患儿的SAI-治疗依从性评分明显高于对照组(均P<0.05);干预后,两组重症肺炎患儿的用力肺活量(FVC)、第一秒用力呼气容积(FEV1)均高于同组干预前,且研究组患儿高于对照组(均P<0.05);研究组患儿的并发症发生率低于对照组患儿(4.44%vs.17.77%,P<0.05)。结论 PICU住院重症肺炎患儿实施全人照护管理模式护理,有利于提高患儿的治疗依从性,改善其肺功能,有效缩短住院时间,并降低并发症的发生率。Objective To explore the effect of holistic care management mode on treatment compliance and pulmonary function in children with severe pneumonia in pediatric intensive care unit(PICU).Methods 90 children with severe pneumonia admitted to PICU of the Children's Hospital Affiliated to Soochow University from January to December 2023 were selected.They were randomly divided into the study group(n=45)and the control group(n=45).The control group received conventional nursing interventions,while the study group received a holistic care mode.The time required for symptom resolution,length of hospital stay,treatment compliance and pulmonary function were compared between the two groups.Results The study group showed significantly shorter hospital stay and time required for symptom resolution compared to the control group(P<0.05).After the intervention,the score of treatment compliance in the two groups were higher than before(P<0.05),with the study group exhibiting significantly higher scores than the control group(P<0.05).After intervention,the forced vital capacity(FVC) and forced expiratory volume in one second(FEV1) were significantly improved in both groups compared to pre-intervention levels(P<0.05),The complication rate in the study group(4.44%)was significantly lower than that in the control group(17.77%)(P<0.05).Conclusions The implementation of holistic care mode in PICU can effectively improve treatment compliance,enhance pulmonary function,effectively reducethe length of hospitalization,and reduce the incidence of complications.
