狒狒综合征1例

摘　　要：报告1例狒狒综合征。患者女,58岁,因左侧胸腹部、双侧肘窝、腋窝等红斑伴瘙痒4 d就诊。患者5 d前曾接触破碎体温计,既往有含汞成分“红药水”接触过敏史。皮肤科检查:左侧乳房及左腹部可见大片状鲜红色斑,边界清楚;双侧肘窝、腋窝及腹股沟可见对称分布暗红色斑,边界尚清,压之褪色,触之皮温稍高,未见糜烂、渗出。实验室及辅助检查:血常规,肝、肾功能、血糖及电解质均正常。乙型肝炎表面抗原(HBsAg)(-)、乙型肝炎表面抗体(HBsAb)(+)。心电图检查正常。诊断:狒狒综合征。予口服依巴斯汀片10 mg,每早1次;盐酸西替利嗪片10 mg,每晚1次;外用炉甘石洗剂(每日4~5次),均用药7 d后停药。治疗第4天时皮损逐渐消退,伴脱屑,8 d后皮损完全消退,部分遗留色素沉着,10 d后色素沉着消退,皮肤恢复正常。目前仍随访中。A case of baboon syndrome is reported.A 58-year-old woman presented with a 4-day history of pruritic erythematous rashes on the left chest and abdomen,bilateral antecubital fossae,and axillae.The patient contacted a broken thermometer 5 days ago.She also had a history of contact allergy to a mercury-containing topical agent known as″red medicine″.Dermatological examination revealed a large demarcated fresh patch on the left breast and abdomen.Symmetrically distributed dark red patches with clear boundaries were noticed in the bilateral antecubital fossae,axillae,and groins.The patches were blanched on pressure,and slightly warm could be felt.No erosions or exudation were observed.Laboratory examinations showed normal routine blood tests,liver and kidney function tests,blood glucose,and electrolyte levels.Hepatitis B surface antigen(HBsAg)was negative,while hepatitis B surface antibody(HBsAb)was positive.Electrocardiogram(ECG)was normal.Diagnosis:Baboon syndrome.The patient was treated with oral ebastine tablets(10 mg once daily in the morning),oral cetirizine hydrochloride tablets(10 mg once daily in the evening),and topical calamine lotion(4-5 times daily)for 7 days.The skin lesions gradually subsided on the 4th day of the treatment,accompanied by desquamation,and the lesions completely resolved,with some residual hyperpigmentation by the 8th day.By the 10th day,the skin appeared normal.The patient is still under follow-up.
