摘 要:目的:比较冻胚移植周期中移植短效长方案来源卵裂期胚胎与囊胚期胚胎对妊娠结局的影响。方法:回顾性分析2014年5月至2023年5月在某医院生殖医学中心接受冻融胚胎解冻移植助孕,且胚胎来源控制性超促排卵(controlled ovarian hyperstimulation, COH)方案为黄体中期短效长方案,内膜准备方案为人工周期方案的1 243例患者的临床资料,根据移植胚胎期别不同分为两组。A组:移植胚胎均为卵裂期胚胎的患者1 136例;B组:移植胚胎均为囊胚期胚胎的患者107例。比较两组患者的年龄、不孕年限、体重指数(body mass index, BMI)及移植日内膜厚度、移植胚胎数、胚胎着床率、临床妊娠率、异位妊娠率、早期流产率等指标。结果:(1)两组患者年龄、不孕年限、BMI、移植日内膜厚度间的差异未见明显统计学意义(P>0.05);(2)A组患者平均移植胚胎数(1.97±0.16)枚大于B组(1.93±0.26)枚,差异有统计学意义(P=0.004);(3)两组患者的胚胎着床率(A、B组分别为37.81%、61.17%、)、临床妊娠率(A、B组分别为55.81%、75.70%、)、早期流产率(A、B组分别为14.83%、6.17%)、多胎妊娠率(A、B组分别为33.44%、55.56%)比较差异有统计学意义(P<0.05);(4)两组患者异位妊娠率比较差异无统计学意义(P>0.05)。结论:对于短效长方案来源胚胎行解冻移植,移植囊胚期胚胎临床妊娠率高,早期流产率低,但多胎妊娠率高,冻胚解冻移植时应优先移植囊胚期胚胎,但应注意移植胚胎个数,以降低多胎妊娠率。Objective: To compare the effects of cleavage stage embryos and blastocyst stage embryos which derived from short-term long protocol on pregnancy outcomes in frozen embryo transfer cycles. Methods: A retrospective analysis was conducted on the clinical data of 1 243 patients who underwent freeze-thaw embryo transfer at the reproductive medicine center of the second affiliated hospital of Zhengzhou university from May 2014 to May 2023. The embryos derived from short-term long protocol and the endometrial preparation program was an artificial cycle program. The patients were divided into two groups based on the different stages of embryo transfer: group A: 1 136 patients who received embryo transfer at the cleavage stage, and group B: 107 patients who received embryo transfer at the blastocyst stage. Compare the age, infertility duration, body mass index, endometrial thickness on the day of transplantation, number of transplanted embryos, embryo implantation rate, clinical pregnancy rate, ectopic pregnancy rate, early miscarriage rate, and other indicators between two groups of patients. Results:(1) There was no significant statistical difference in age, infertility duration, BMI, and endometrial thickness on the day of transplantation between the two groups of patients(P>0.05);(2) The average number of transplanted embryos in group A(1.97±0.16) was higher than that in group B(1.93±0.26), and the difference was statistically significant(P=0.004);(3) There was a statistically significant difference(P<0.05) in the embryo implantation rate(37.81%, 61.17%, respectively), clinical pregnancy rate(55.81%, 75.70%, respectively), early miscarriage rate(14.83%, 6.17%, respectively), and multiple pregnancy rate(33.44%, 55.56%, respectively) between the two groups of patients;(4) There was no statistically significant difference in the ectopic pregnancy rate between the two groups of patients(P>0.05). Conclusion: For freeze-thaw embryo which derived from short-term long protocol transfer, the clinical pregnancy rate is high
