摘　　要：目的:根据锥形束CT(conebeam CT, CBCT)重建影像研究开封地区下颌第一恒磨牙远中舌根,统计其发生率和不同方向的弯曲角度。方法:选择患者的CBCT图像数据,统计分析下颌第一恒磨牙远中舌根的发生率。用Schneider法测量远中舌根的弯曲度。结果:筛选出628例符合标准的下颌第一恒磨牙病例中,40.76%的人群有下颌第一恒磨牙远中舌根。1 256颗牙中远中舌根发生率为33.76%。右侧发生率比左侧高,左、右侧的发生率差异有显著意义(P<0.05)。近远中向和颊舌中向测得的远中舌根平均弯曲度分别为(25.62±10.51)°和(19.71±12.62)°。近远中向的弯曲度大于颊舌向观,有显著性差异(P<0.001)。结论:了解开封地区下颌第一磨牙远中根的根管系统,可以减少遗漏根管的发生,为预防根管预备过程中并发症提供理论支持。Objective: To investigate the incidence of mandibular three-rooted permanent first molars, and their curvature in various directions in Kaifeng, as observed through CBCT images. Methods: CBCT image data from patients were selected for statistical analysis of the rate of distolingual roots of mandibular first permanent molars, and the maximal curvature angle of the distolingual roots was measured. Results: Among the 628 cases of mandibular first permanent molars that met the inclusion criteria, 40.76% of the population had a distolingual root of mandibular first permanent molars.The incidence in 1 256 teeth was 33.76%. Higher incidence on the right than on the left.It makes a huge difference in the incidence of distolingual roots between the occurrence of right and left lateral dentition(P<0.05). The mean curvature of distolingual roots in the buccolingual view measured by Schneider's method was(19.71±12.62)°,while in the proximal view, the average curvature was(25.62±10.51)°, and the curvature was larger in the proximal view than in the buccolingual view. There is a significient statistical difference(P<0.001). Conclusion: Understanding the distolingual roots of mandibular first molars in Kaifeng area can reduce the occurrence of missed root canals, and provide theoretical support for the prevention of complications in the process of root canal preparation.
